摘  要:背景:重度下肢畸形矫正是骨科教学的难点,引入3D打印技术及虚拟现实(VR)技术,有望弥补传统教学模式的不足,提高教学效果。目的:探讨通过3D打印技术和VR技术,增强青年医师对重度下肢畸形特点的理解及对多段截骨矫正技术的掌握,改革教学模式,提高教学效果。方法:选取我科2018年11月至2019年5月收治的重度下肢多根长骨畸形并接受长骨多段截骨矫形内固定的患者6例,对其影像通过Mimics 15.0软件行三维重建,打印全下肢3D模型并构建术中虚拟模型。同期在骨科进行规范化培训的外科住院医师及实习医师共42人,随机分为对照组和试验组,每组各21人。对照组依靠查体、X线及CT检查的传统教学模式,讲解畸形特点、多段截骨顶点设计等教学内容。试验组在此基础上,采用3D打印模型及VR虚拟模型等手段辅助教学。课后通过主观问卷及临床考核判断教学效果。结果:试验组在主观问卷调查自我评价得分和临床考核得分方面,均显著优于对照组,差异具有统计学意义(P<0.05)。结论:在传统教学模式基础上结合3D打印技术及VR虚拟演示,可提高学生对复杂畸形特点的理解和对多段截骨技术的掌握,弥补传统教学的不足,具备更好的教学效果。Background:The correction of severe lower limb deformity is a difficulty of orthopedic teaching.The 3D printing and virtual reality(VR)techniques are expected to make up for the shortcomings of traditional teaching mode and improve teaching efficiency.Objective:To explore the value of 3D printing and VR in teaching the features of severe lower limb deformity and multi-segment osteotomy.Methods:Six patients with severe multiple long bone anomalies of lower extremity treated between November 2018 and May 2019 were enrolled in this study.Mimics 15.0 software was used to perform three-dimensional reconstruction of their images.3D model of the whole lower limbs was printed,and intraoperative virtual model was constructed.Forty-two house surgeons and interns who received standardized training in orthopedic department were randomly divided two groups(n=21):control group and experimental group.The content of courses,such as deformity characteristics and apex design in multi-segment osteotomy were illustrated by physical examination,X-ray and CT scanning in the control group.In the experimental group,assisted teaching means,such as 3D printing model and VR model were used additionally.Teaching effect was evaluated by subjective questionnaire survey and clinical examination after class.Results:Both the scores of subjective questionnaire survey and clinical examination in the experimental group were significantly higher than those in the control group(P<0.05).Conclusions:Traditional teaching mode in combination with 3D printing model and VR demonstration can improve students’understanding for the features of complex anomalies and the technique of multi-segment osteotomy,make up for the shortcomings of traditional teaching,and achieve better teaching results.
