摘 要:目的 :观察每日过夜戴用OK镜 1年后角膜内皮细胞的形态及数量的变化。方法 :77例 15 2只眼每日夜间戴用OK镜 (Dk/L值 6 6 ) 7~ 10小时 ,分别于戴镜前及戴镜后 1、 3、 6和 12个月时用非接触式角膜内皮显微镜观察中央区内皮细胞密度、细胞大小的变异系数及六角形细胞百分率。结果 :戴镜后 1月时细胞密度与戴镜前无差异 (P =0 4 86 ) ;戴镜后 3月较戴镜前和戴镜后 1月时分别增加 1 7% (P =0 0 0 4 )和 2 2 % (P =0 0 0 2 ) ;6月与 3月时相当 (P =0 4 31) ,但较戴镜前增加 1 9% (P =0 0 2 5 ) ;12月时较 6月时降低 2 0 % (P =0 0 0 1) ,回落至戴镜前的水平 (与戴镜前比较 ,P =0 30 7)。变异系数于 6月时较 3月时降低 4 9% (P =0 0 19) ,12月时较6月时增加 4 1% (P =0 0 31) ,但各时点与戴镜前相比差异无显著性。六角形细胞百分率戴镜前后及戴镜后各时点间均无差异。结论 :OK镜夜戴一年 ,未发现角膜内皮细胞形态及数量的负性变化 ;戴镜后 3月及 6月时细胞密度的增加与OK镜所致角膜形状改变引起的旁中央或周边区细胞向中央区移行有关。Objective:To investigate changes in the human corneal endothelium after long-term nocturnal orthokeratology lens ear.Methods:Seventy-seven subjects(152 eyes)wore orthokeratology lens(BE,Capricornia Inc.Brisbane,Australia.DK/L=66)during sleeping for 7~10 hours each night for 1 year.Prefitting and postfitting specular microscopy of the central corneal endothelium was performed without discontinuation of orthokeratology lens wear.The central endothelium was analyzed for cell density,coefficient of variation in cell size(CV),and hexagonality.The prefitting data were compared with those obtained 1,3,6,and 12 months after the beginning of orthokeratology lens wear.Results:The postfitting mean cell density was found that these was no change at 1-month follow-up(P=0.486),but was increased significantly at 3-(1.7%,P=0.004)and 6-months(1.9%,P=0.025)follow-ups,and then was decreased to the prefitting level at 12-months follow-up.The coefficient of variation was decreased at 6-month by 4.9% when compared with that of 3-months follow-up(P=0.019),and was increased at 12-months by 4.1% when compared to that of 6-months follow-up(P=0.031),but no significant differences in CV were found between prefitting and postfitting values.No significant differences in hexagonality were found between prefitting and postfitting values and when the postfitting values were compared with each other.Conclusion:In this pilot study,there was no negative changes in mean cell density and morphometric indexes 1 year after nocturnal orthokeratology lens wear.The transient increase in mean cell density of central endothelium might be explained by the redistribution of corneal endothelial cells induced by the changes of the corneal shape after orthokeratology lens wear.
