摘 要:目的 探讨nm2 3基因异常表达及突变与结直肠癌侵袭、转移之间的关系。方法 应用免疫组化LDP法、流式细胞术、PCR SSCP法对 1 5 0例结直肠癌标本及正常黏膜组织和 1 6例大肠腺瘤分 3组进行nm2 3蛋白检测和基因突变的筛查。结果 ①免疫组化发现 73例结直肠癌中nm2 3 H1蛋白表达水平低于正常黏膜组织和良性腺瘤 (P <0 0 0 1 ,P <0 0 0 1 ) ,转移癌表达低于原发癌 (P <0 0 0 1 ) ,结直肠癌nm2 3 H1蛋白表达与组织分化、肠壁浸润、Dukes分期、转移均密切相关 (P <0 0 0 1 ) ,低表达组术后预后差于高表达组 (P <0 0 0 1 ) ;②流式细胞术研究表明 ,nm2 3 H1蛋白在癌组织表达低于癌旁组织 ;在结直肠癌伴有转移 (3/ 1 4 )时其表达低于未发生转移者 (1 1 / 1 4 ) ;③nm2 3 H1基因第 2~ 5位外显子在 6 3例结直肠癌组织中未发现异常突变。结论 结直肠癌中nm2 3基因突变可能是小概率事件 ,在结直肠癌的侵袭、转移中不发挥主要作用 ,而nm2 3 H1蛋白表达降低则与结直肠癌的侵袭、转移有关 ,提示检测nm2 3蛋白表达是判断结直肠癌预后有价值的指标之一。Purpose To investigate the role of nm23 gene mutation and the expression of nm23 gene product in the invation and metastasis of colorectal cancer. Methods The expression level of nm23 H1 protein and the mutation of nm23 H1 were detected from 150 Chinese colorectal carcinomas, matched normal mucosa and 16 adenomas by immunohistochemistry (LDP methed), flow cytometry (FCM) and polymerase chain reaction (PCR) single stranded conformation polymorphism (SSCP). Results (1)The positive rate of nm23 H1 protein expression in colorectal carcinoma was significantly lower than that in adjacent normal mucosa and adenoma ( P <0 001, P <0 001). In metastatic cancer, it was significantly lower than that in primary cancer( P <0 001) There were significant differences between nm23 H1 protein positivity and colorectal carcinoma histological type,depth of invasion, Dukes term and the tumour metastasis ( P <0 001). The 5 year survival rate of patients with high expression of nm23 H1 protein was significantly higher than that with low expression of the protein. (2)nm23 H1 protein expression level was lower in colorectal tumours than that in matched normal mucosa by FCM, and in 3 cases of metastasis were significantly different from those in colorectal cancer. (3)No mutation in the exons 2,3,4 and 5 of nm23 H1 in 63 colorectal cancer samples was detected. Conclusions nm23 H1 mutation may be a rare event in colon tumors. nm23 H1 gene mutation do not play an important role in the invasion and invasion and metastasis of colon tumors. Howerver, the decreased expression of nm23 H1 protein may be a key cause of colorectal carcinoma metastasis. Determination of nm23 H1 protein is the most reliable to predict metastasis and prognosis in patients with colorectal cancer.
