扫描电镜对卵巢上皮性肿瘤的诊断价值

摘　　要：本文用扫描电镜(SEM)观察了卵巢不同类型的上皮性肿瘤42例。各类型肿瘤有不同表面形态特征,对判断肿瘤的组织类型有重要意义。SEM对分辨和发现癌细胞亦有很大优越性。有诊断意义的癌细胞表面形态特征为:(1)出现奇形乳头或奇形细胞簇;(2)可见奇形细胞或巨细胞;(3)细胞重度多形性及排列紊乱、拥挤,致被复面崎岖不平;(4)存在细胞的特异性分化不良。本组有3例临界恶性瘤在SEM下发现光镜所不能辨认的电镜下局部细胞癌变。6例恶性Brenner氏瘤的扫描电镜观察进一步证明肿瘤为鳞状上皮所构成,发生自苗勒氏管上皮。45 cases of different types of ovarian epithelial tumors were studied by scanning electron micro-scopy (SEM). The surface morphologic features varied in defferent types of tumors. It possesseddiagnostic significance for determing the histologic types of tumors. SEM also has a great advantageto distinguish and discover the cancer cells. The surface morphologic features possessing diagnosticsignificance for cancer cells were determined: 1. Appearance of bizarre papillae or clusters;2.finding of bizarre cells or giant cells; 3. Servere polymorphism and disturbance of cell arrangmentand crowd of cells forming a rugged surface structure; 4. Presence of the less differentiated specificstructures of cells. In 3 cases of borderline malignancy tumors of present study the submicroscopicchanges of local cell malignancy were found by SEM, but could not be distinguished by light micro-scopy.The SEM observations of malignant Brenner tumors further evidenced the tumor consisting ofsquamous cells, which originated from Mullerian epithelium recapitulating the epithelium of vagino-cervix.
