摘　　要：目的　探讨包膜期脑脓肿MRI表现。方法　经手术或临床抗炎证实的包膜期脑脓肿 3 1例 ,男 18例 ,女 13例 ,年龄 3～75岁。 2 8例行平扫加增强扫描 ,3例仅行平扫。结果　所有病例脓肿壁上均有一光滑的低信号“暗带” ,增强扫描见脓肿壁呈厚度均匀的环形强化 ,9例有“多房征” ,10例有脑实质外播散 (即脑脓肿累及脑室内和 /或硬脑膜和 /或蛛网膜下腔 )。结论　包膜期脑脓肿的MRI表现有一定特征 ,其中“暗带”最具特征 ,其次为环状强化 ,“多房征”及脑实质外播散表现。Objective To analyze MR findings of brain abscess in capsule-period. Methods Thirty-one case(18 males,13 females,age ranged from 3 to 75 years)of brain abscess in capsule-period were collected,which were verified by surgery or anti-inflammatory treatment.Pre-and post-contrast MRI was performed in 28 cases,precontrast MRI was done in 3 cases.Results A smooth low-signal zone in abscess wall was seen in all cases on plain MRI,a homogeneous ringlike enhancement was seen in all enhanced cases.The “multi-locular sign”was seen in 9 cases,extraparenchymal spread(intraventricular and/or dura mater and/or subarachnoid)was detected in 10 cases.Conclusion Brain abscess in capsule-period has a specific MR finding,especially the low-singal zone in the wall,the others are ringlike enhancement,“multi-locular sign”and extraparenchymal spread.
