海藻酸钠-聚赖氨酸-海藻酸钠-微囊化牛肾上腺嗜铬细胞镇痛微胶囊镇痛过程中的不良反应:初步观察

摘　　要：目的:评价海藻酸钠-聚赖氨酸-海藻酸钠(APA)微囊化牛肾上腺嗜铬细胞BCC移植于患者蛛网膜下腔后的不良反应。()方法:采用腰椎穿刺术,将微胶囊按0.5×107,1.0×107,1.25×107,1.5×107/次·人4个剂量组,注入癌痛患者蛛网膜下腔内。结果:全组发生I,II度或轻度不良反应14例70%),其中腰腿酸痛((65%)、头晕(50%)、头痛(35%)、发热(25%)、乏力(30%)、恶心(5%)、心悸(5%),持续中位时间在0.5×107,1.0×107剂量组多为两三天,1.25×107,1.5×107剂量组多为4～11d;发生重度不良反应2例(10%),均为1.5×107剂量组,分别表现为双下肢频发抽搐、肩背部压迫感,持续2～5d,无需特殊处理可自愈。移植后患者的生命体征、肝肾功能、免疫功能均无明显改变。结论:APA-BCCs微胶囊蛛网膜下腔移植具有操作简便、不良反应轻、安全的特点,尤其剂量低于1.5×107/次·人更为安全。AIM:To evaluate the adverse reactions of alginate polylysine alginate(APA)microencapsulated bovine chromaffin cells(BCCs) transplanted into the subarachnoid space of cancer patients. METHODS:Lumbar puncture were used, the microcapsules were divided into four dose groups: 0.5×107,1.0×107,1.25×107 and 1.5×107 per time for each person, and were injected into the subarachnoid space of the patients with cancer pain. RESULTS:The adverse reactions of Ⅰdegree,Ⅱdegree or mild were 14cases(70%),including waist leg ache(65%),dizziness(50%), headache(35%),fever(25%),inertia(30%),nausea(5%), palpitation(5%).The media time of these adverse reactions remained 2 to 3 days in 0.5×107 and 1.0×107 dose groups,4 to 11 days in 1.25×107 and 1.5×107 dose groups. Two cases had the serious adverse reactions in 1.5×107 dose group, including the frequent spasms of both lower extremities and the heavy feeling of shoulder back that remained 2 to 5 days,and could be spontaneously cured without special treatments. The vital signs, liver function,kidney function,immune function of these patients had no significant changes. CONCLUSION:APA BCCs xenotransplantation is safe and easy to operate, and has mild adverse reaction, which is safer especially when the dose is less than 1.5×107 per time for each person.
