公立医院坚持公益性的道德困境与改革思考——基于广东省高州市人民医院制度创新的伦理考察

摘　　要：在介绍"高州模式"坚持公益性的制度经验在于改革创新动力、共赢发展理念和有力监督保障机制的基础上,提出从政府、医院、社会三个维度即从政府的角度而言,找准职能定位,引入竞争机制,激发改革动力,是保证公立医院坚持公益性的关键所在;从医院的角度而言,需要树立经济效益和社会效益协调发展的理念,摆脱对"粗放型"发展模式的路径依赖,强化精细化管理;从社会角度而言,引入第三方监管机构,加强医疗卫生领域的行业自律,是规范公立医院医疗行为、保障其公益性的外在条件,进行公立医院制度改革。The experience of ＂Gaozhou＂ model lies in innovation power, win -win development idea and powerful supervision and guarantee mechanism, furthermore this paper proposed the three dimension （government, hospitals, society） for adhering to the public benefits： for the govemment, should get the function orientation, introduce competition mechanism, stimulate reform power, which is the key point; for the hospital, should establish the development idea with balancing economic benefit and social benefit, stop rely on the extensive development model, and strengthen the fine management; for the social, should introduce the third party regulators, strengthen the self- discipline, standard public hospitals＂ clinical practice, safeguard the conditions for public benefits and push the system reform in public hospitals.
