遗传性非息肉病性大肠癌发病率及临床病理特点探讨

摘　　要：目的　研究遗传性非息肉病性大肠癌 (HNPCC)在大肠癌中的比例及临床病理特点。方法　收集和分析 5 5 0例大肠癌的临床病理及随访资料 ,符合Amsterdam标准或日本标准者诊断为HNPCC。结果　 5 5 0例大肠癌中 6例 (1 1 % )符合Amsterdam标准 ,1 9例 (3 5 % )符合日本标准。本组HNPCC的临床病理特点是肿瘤发病年龄早 ,多位于右半结肠 ,粘液癌及多原发大肠癌多见 ,家族中肠外肿瘤增多 ,息肉伴随增多不明显。结论　HNPCC具有特殊的临床病理特点 。Objective To study the frequency and clinicopathologic characteristics of hereditary nonpolyposis colorectal cancer(HNPCC).Methods We analyzed the clinicopathologic and follow-up data of 550 colorectal carcinoma(CRC) patients.All HNPCC patients fulfilled Amsterdam criteria or Japanese criteria.Results Of all 550 CRC patients,6 patients(1 1%) fulfilled Amsterdam criteria,19 patients(3 5%) fulfilled Japanese criteria.HNPCC patients were characterized by the early onset age of CRC,predilection for proximal colon,more mucoid carcinoma and multiple primary carcinoma,less polyps complicated and increasing tumors of extracolorectal site in family members.Conclusion HNPCC had special clinicopathologic characteristics and the frequency of HNPCC here was similar to the foreign reports.
