摘  要:目的探讨结直肠癌中K-ras基因突变对其自身及Ras、EGFR和p53蛋白表达的影响及意义。方法应用免疫组化SP法和实时荧光定量PCR Taqman-ARMS探针法,检测50例结直肠癌和20例正常肠黏膜组织中Ras、EGFR和p53蛋白的表达及K-ras基因的突变情况。结果结直肠癌中Ras、EGFR和p53蛋白表达水平明显高于正常肠黏膜(P<0.05);其K-ras基因突变率为44%,正常组织中未检出突变(P<0.05)。Ras蛋白阳性表达与肿瘤分化程度相关(P<0.05),与浸润深度、病理分期、淋巴结转移无关;EGFR蛋白表达与浸润深度相关(P<0.05),与分化程度、病理分期、淋巴结转移无关;p53蛋白表达与淋巴结转移、病理分期相关(P<0.05),与分化程度、浸润深度无关。结直肠癌组织中K-ras基因突变与Ras蛋白表达呈负相关性(P<0.05),与EGFR、p53蛋白表达无相关性;EGFR与Ras蛋白的表达具有正相关性(P<0.05)。结论 K-ras基因突变及Ras、EGFR、p53蛋白高表达均为结直肠癌发生的重要分子事件,与肿瘤分化、浸润及转移密切相关,对其进行联合检测有助于结直肠癌的预后评估及靶向治疗药物的选择。Purpose To investigate the expression of Ras,EGFR and P53 proteins and mutation of K-ras gene in the colorectal carcinoma and to study their clinical significance.Methods The expression of Ras,EGFR and P53 proteins in 50 cases of colorectal carcinoma and 20 cases of normal colorectal mucosa was detected by SP immunohistochemical method and K-ras gene point mutations were analyzed by real-time fluorescence quantitative polymerase chain reaction.Results The positive expression rates of Ras,EGFR and P53 protein in colorectal carcinoma were markedly higher than that in normal tissues (All P < 0.05).The mutation rate of K-ras gene was 44%.The expression of Ras,EGFR and P53 proteins had positive correlation to the cell differentiation,depth of invasion and lymph node metastasis (All P < 0.05),respectively.There was negtive correlation between the expression of EGFR protein and mutation of K-ras gene,and positive correlation between the expression of EGFR and Ras protein in the colorectal carcinoma.Conclutions The mutations of K-ras gene and overexpression of Ras,EGFR and P53 protein are closely related to the pathogenesis and development in the colorectal carcinoma.Combined detection will be helpful for improving evaluation in the diagnosis and prognosis of colorectal carcinoma and developing the individualized cancer treatment.
