摘  要:目的对一起由旅游团聚餐引起的食物中毒事件进行调查和分析,探索致病因素并提出防控措施。方法制定病例定义开展病例搜索,根据发病流行曲线推断可疑暴露餐次,通过病例对照研究分析可疑食物。分析发病与进食量的剂量反应关系、潜伏期和剂量的关系。开展现场卫生学调查,采集相关样品进行实验室检测。结果本次中毒确诊病例14名,均为同一旅游团旅客。病例对照研究显示2017-08-14晚餐的"花斑鱼"OR=73. 67,95%CI:6. 85~792. 46,病例组的进食量高于对照组(OR=16. 2,95%CI:2. 61~100. 45),发病与进食量有剂量反应关系,剂量与发病潜伏期呈负相关关系(r=-0. 573,P<0. 05)。1份养殖水槽水和6份来病例的肛拭子检出副溶血性弧菌。结论这是一起副溶血性弧菌污染引起的细菌性食物中毒。危险餐次为2017-08-14晚餐,危险食物为"花斑鱼"。建议加强对旅游团餐的监管,控制酒店接待上限、规范加工流程,防止类事件再次发生。Objective To investigate and analyze a case of food poisoning related to a dinner in a tour group,explore the pathogenic factors and put forward measures for prevention and control. Methods Case definition was established to conduct case search,suspected exposure dinner was inferred according to disease prevalence curve,and suspected food was identified through a case control study. The dose-response relationship between disease and intake amount and the relationship between latency and doses were analyzed. A field sanitary investigation was conducted,and related samples were collected for laboratory testing. Results Poisoning was confirmed in 14 tourists from the same tour group. The case-control study showed that the "spotted fish"OR =73. 67( 95%CI: 6. 85-792. 46) in the dinner on August 14,2017. The food intake was greater in the case group than in the control group( OR = 16. 2,95%CI: 2. 61-100. 45);there was a dose-response relationship between disease development and food intake amount. The dose was negatively correlated with the latency( r =-0. 573,P<0. 05). Vibrio parahaemolyticus was detected from one aquaculture flume water and six anal swabs.Conclusion This is a bacterial food poisoning case caused by Vibrio parahaemolyticus contamination. The dinner responsible was the dinner on August 14 and the food responsible was "spotted fish". It is suggested supervision of tour group meals be enhanced, the limit of hotel reception be controlled and the processing flow be standardized so as to prevent recurrence of similar events.
