摘 要:目的 了解上海市全科医师工作现状,探讨改善全科医师队伍建设的对策.方法 以上海市中美全科医学培训交流中心为依托,对220位全科医师进行问卷调查,内容包括人口学信息、工作现状、工作满意度等.结果 发放问卷220份,回收有效问卷187份,有效问卷回收率85.0%.受调查医师中参与全科医学转岗培训的有103人,占比55.1%.本科及以下学历者为138人,占比73.8%.中级及以下职称者为127人,占比67.9%.受调查医师选择从事全科医学工作的主要原因是“看好全科医学发展前景”.工作压力主要来源于“医患关系紧张”,其次为“职称晋升压力”.根据全科医学工作年限分层之后,不同组别受调查医师对收入满意度、工作压力和工作满意度的测评差异具有统计学意义(P<0.05).因子分析结果表明,累计方差贡献率为65.12%,影响全科医师工作满意度的主要因素为职称、月收入、工作压力和工作强度.结论 尽管绝大多数受调查医师对全科医学发展前景持肯定态度,但当前的职业满意度不尽理想.切实改善全科医师工作现状,提高年轻医师待遇并完善保障机制,是建设高素质全科医师队伍的重要环节.Objective To understand the current status of general practitioners in Shanghai,and to explore strategies for job satisfaction improvement.Methods 220 general practitioners were surveyed with a questionnaire collecting data on areas such as demographic information,current work situation,and job satisfaction.Results 187 valid questionnaires were received out of 220,with an 85.0% response rate.As a result,55.1% (103) respondents had participated in a retraining program due to a job transfer.It also shows that 73.8% (138) respondents have a bachelor degree or below;while 67.9% (127) are in primary or intermediate level.The major reason they choose general medicine is for positive career prospects.Work stress is mainly originated from " tension from doctor-patient relationships" and "promotion pressure".The respondents are categorized into three different groups,which are based on their years of service.The measurement differences in income satisfaction,job stress,and job satisfaction in the different groups have statistical significance (P〈 0.05).Data analysis shows that the cumulative variance contribution rate is 65.12%.The main influencing factors of job satisfaction are titles,monthly incomes,work pressure,and work intensity.Conclusions Although most respondents are positive about the future of their work,actions need to be made to improve their job satisfaction.It is essential to improve the current situation,compensation,and employee benefits for general practitioners.
