利用TCGA数据集分析H-ras、mTOR基因在膀胱癌中的表达及临床意义

摘  要:目的:研究H-ras、mTOR基因与膀胱癌发展、预后的关系及其通路H-ras/mTOR存在的可能性。方法:从美国癌组基因组图谱(TCGA)数据库中下载膀胱癌中H-ras、mTOR基因表达谱及其临床信息资料并分析其关系,分析H-ras、mTOR基因表达的相关性。结果:(1)AJCC p TNM分期中T_1~T_4期(P=0.012)、T_2~T_4期(P=0.011)、T_(1~2)~T_(3~4)期(P=0.042)及病理分级(P=0.020)与H-ras的表达呈负相关(P<0.05),但与膀胱癌的肌层浸润、转移、预后无明显相关(P>0.05)。(2)AJCC p TNM分期中T_2~T_3期(P=0.042)及病理分级(P=0.014)与mTOR的表达正相关(P<0.05),mTOR低表达的患者无瘤生存期明显长于高表达组(P<0.05)。但mTOR与膀胱癌的肌层浸润、转移无明显相关(P>0.05)。(3)COX多因素分析表明mTOR是影响膀胱癌预后的独立因素。(4)H-ras、mTOR基因在膀胱癌中表达具有显著正相关(P<0.05)。结论:H-ras基因可作为膀胱癌恶性程度的良性预示因素,而mTOR基因是膀胱癌恶性程度的不良预示指标,并且是影响预后的独立危险因素。两者可能通过H-ras/mTOR信号通路联系而发挥肿瘤生物学作用并有望成为膀胱癌靶向治疗的理想位点。Objective: To study the relationship between H-ras, mTOR gene and the prognosis of bladder cancer, and the possibility of the existence of H-ras/mTOR pathway.Method: The Gene expression profiles of H-ras and mTOR in bladder cancer and their clinical data were downloaded from the American Cancer Genome Atlas ( TCGA ), the relationship between H-ras, roTOR and clinical data', the correlation between the expression of H-ras and mTOR genes were analyzed.Result: ( 1 ) The expression of H-ras was negatively correlated with AJCC pTNM stage T1-T4 ( P=0.012 ), T2-T4 ( P=0.011 ), T1-2-T3_4 ( P=0.042 ) and pathological grading ( P=O.020 ), but the invasion and metastasis prognosis weren't associated with the level of H-ras ( P〉0.05 ) . (2) The expression of roTOR was positively correlated with AJCC pTNM stage T2-T3 (P=0.042), and pathological grading ( P=0.014 ) .The patients with low expression of mTOR had longer tumor free survival time than the high expression group ( P〈0.05 ) .But there was no significant correlation between mTOR and the muscle invasion and metastasis of bladder cancer (P〉0.05 ) . ( 3 ) COX multivariate analysis showed that mTOR was an independent prognostic factor for bladder cancer. ( 4 ) The expressions of H-ras and mTOR gene in bladder cancer were significantiy positively correlated (P〈0.05) .Conclusion: H-ras gene can be used as a benign prognostic factor in the malignant degree of bladder cancer, and mTOR gene is an indicator of the malignant degree of bladder cancer.They may play a role in tumor biology through the H-ras/mTOR pathway.
