摘 要:目的 探讨班主任负责的年级制教学模式对提高急诊科新入职护士核心能力的影响.方法 采用目的抽样法,选取2004年1月—2008年12月北京协和医院急诊科新入职护士45名为对照组, 2009年1月—2013年1月新入职护士52名为试验组.对照组护士采用急诊科传统培训方法,试验组护士采用班主任年级制教学方法.采用注册护士核心能力量表(CIRN)评估两组护士培训3年后的核心能力,培训3年后比较两组护士理论和技能考核成绩.结果 试验组护士CIRN总分为(191.35±25.43)分,对照组为(162.58±29.27)分,两组比较差异有统计学意义(t=-5.181,P〈0.01);试验组护士专科护理操作考核成绩为(8.79±0.78)分,急救护理技术操作为(8.69±0.67)分,高于对照组的(7.13±0.87)、(7.67±0.95)分,差异有统计学意义(t值分别为-9.908、-6.022;P〈0.01).结论 班主任负责的年级制教学模式可以有效提高急诊科新入职护士的核心能力和护理操作水平.Objective To explore the effects of head teacher grade system teaching on improving core capability in new recruited nurses of emergency department. Methods According to purposive sampling, a total of 45 new nurses recruited from January 2004 to December 2008 in the Department of Emergency of Peking Union Medical College Hospital were selected into the observation group and 52 nurses recruited from January 2009 to January 2013 were selected into the control group. The control group were trained using conventional method while the observation group received head teacher grade system teaching. The Competency Inventory for Rregistered Nurse Scale (CIRN) was used to evaluate the core capability between two groups after 3 years of training. The theory and practice skills were compared between the two groups after 3 years of training. Results The total scores of CIRN were significantly higher in the observation group compared with the score in the control group [(191.35±25.43) vs. (162.58±29.27); t=-5.181; P〈 0.001]. The observation group had significantly higher scores of practice skills and emergency nursing skills compared with the control group [(8.79±0.78) vs. (7.13±0.87) for practice skills; (8.69±0.67) vs. (7.67±0.95) for emergency nursing skills;t=-9.908, -6.022; P〈0.01]. Conclusions The head teacher responsible grade system teaching mode can effectively improve the core capability and clinical nursing skills in new nurses of emergency department.
