住院医师规范化培训层级管理模式探讨

摘　　要：住院医师规范化培训是医学生毕业后教育的重要组成部分,是实现"健康中国"、分级诊疗的根本政策,只有建立一整套科学合理的住培管理体系,才能保障住培工作的质量内涵建设,从而培养出高素质、德才兼备的住院医师。中南大学湘雅医院紧跟政策导向,结合实际情况,多年来探讨并实施了住院医师规范化培训层级管理模式。本文介绍了住院医师规范化培训层级管理模式的产生背景、现状、实践与成果,比较分析了既往管理模式和新管理模式下学员管理、培训课程制定、师资管理等方面的差异,发现新的层级管理模式能使学员的培养方案、培训课程更科学、合理,提高了各级师资的积极性,同时提高了学员的培训效果,并且使住培管理实现了从宏观粗放到微观精细的跨越,管理层次分明,指导重心下移,管理效率大幅提高。住院医师规范化培训层级管理模式能有效提高住院医师培训的管理效率,有其实施与推广的价值和意义。The standardized residency training is an important part of postgraduate medical education for medical students. It is also the fundamental strategy to achieve the "Healthy China" and the hierarchical health system. The establishment of a scientifically reasonable standardized residency training system is the guarantee of high-quality residents with professional competence and political integrity. Attention is increasingly payed to the standardized residency training programs by the Chinese government. Xiangya Hospital of Central South University followed the government's policies and is exploring the stratified management system of standardized residency training suitable to the local residency training bases. This article introduced the background, current conditions, practice and results of the stratified management system of standardized residency training, through analysis and comparisons of the resident management, curriculum design, and instructor management under the new training patterns and the old patterns. The study discovered that the new patterns had a more scientific and reasonable training plans and curriculums, which can hugely enhance the motivation of all residents and improve the training results of the trainees. Also, the new patterns had shifted the residency management style from the extensive macro-management to the intensive micro-management. The management levels are clearly defined and residency managers and instructors at all levels perform their own functions. The management efficiency is largely increased. Thus the stratified management mode of the standardized residency training can effectively improve the management efficiency and deserve the implementation and promotion.
