摘  要:目的了解1起食源性疾病暴发事件发生的危险因素和致病因子,为防止今后类似事件的发生提供参考依据。方法 对患者进行病例对照研究,用SPSS 13.0统计处理,对2018年6月11日在槐荫区某中学发生的1起食源性疾病暴发事件进行现场流行病学调查,并采集部分病例粪便标本、肛拭子病毒检测,全部留样食品样本进行常见致病菌检测。结果事件中首例病例发病时间为6月11日16:30,末例病例发病时间为6月12日06:15,共报告病例21例,教师罹患率为19.09%(21/110),学生未搜索到相关病例,无续发病例。病例临床表现以腹泻(20例、占95.24%)、腹痛为主(18例、85.71%),偶有恶心(4例、占19.05%)、呕吐(1例、占4.76%)、腹胀(3例、占14.29%)等症状。病例对照研究显示食用西红柿炒鸡蛋的发病风险显著增加(OR=7.61)(P<0.05)。实验室检测结果显示,留样食品中西红柿炒鸡蛋检出嗜水气单胞菌。结论该事件为1起变质鸡蛋污染菜品,导致嗜水气单胞菌感染引起的食源性疾病暴发疫情。Objective To understand the risk factors and pathogenic factors of a foodborne disease outbreak,so as to provide the reference for the prevention of similar events in the future. Methods A case-control study was conducted to the patients,and statistical treatment was applied by software SPSS 13.0.Field epidemiological investigation was conducted on the foodborne disease outbreak in a middle school in Huaiyin district on June 11 2018.Fecal specimens and anal swabs of some cases were collected for virus detection,and common pathogenic bacteria were detected for food samples. Results The onset time of the first case was 16:30on June 11,and it of the last case was 06:15on June 12.A total of 21cases were reported,with the attack rate of 19.09%(21/110)among teachers,no relevant case was found among students,and no secondary case was found.The clinical symptoms were mainly diarrhea(20cases,95.24%)and abdominal pain(18cases, 85.71%),few cases had symptoms of nausea(4cases,19.05%),vomiting(1cases,4.76%)and abdominal distension (3cases,14.29%).The case-control study showed a significant increased risk of eating scrambled eggs with tomatoes (OR =7.61,P <0.05).Laboratory test results showed that aeromonas hydrophila was detected in tomato scrambled eggs. Conclusion The event is a foodborne disease outbreak caused by spoiled eggs contaminated with aeromonas hydrophila.
