社区全科医师导师制培养的探索与实践

摘　　要：分级诊疗制度建立的关键是基层首诊。为了推动社区全科医师能力的全面提升,上海市第一人民医院与上海市松江区卫健委合作开展全科医师导师制培养项目。遴选松江区15个社区卫生服务中心共46名骨干医师参加本项目培训,培训以目标学科为主,兼顾全科,三级医院的导师从临床思维、诊疗技术、双向转诊、案例建设、科研能力等5个维度开展"一对一"指导。经过两年培养,学员临床能力得到了较大幅度提升,所在6个社区开展了临床新技术或新项目,双向转诊有序开展。社区医师的导师制培养是国家住院医师规范化培训制度的重要补充,三级医院通过地方卫生主管部门与社区卫生服务中心开展联动培养,是一种有益尝试。The key to the establishment of hierarchical health system is the first diagnosis at the grassroots level,and the basis is that community general practitioners have the abilities of being the "gatekeepers".In order to promote the overall improvement of community general practitioner abilities,Shanghai General Hospital cooperated with the Songjiang District health and Family Planning Commission to carry out the training programs of general practitioner tutorial system in Shanghai.46 backbone physicians of 15 community health service centers in Songjiang District were selected in the project,5 dimensions of clinical thinking,treatment technology,two-way referral,case construction,and scientific research ability were focused by "one to one" instructors from the grade 3 hospitals.At the end of the project,a questionnaire survey was conducted.After 2 years of training,the students' clinical thinking abilities have been greatly improved,new technology or new projects were operated in the 6 communities,two - way referral were conducted in an orderly manner,and some of the doctors applied for research and published papers.Tutor training community physicians is an important supplement to the national standardized residency training system.Grade three hospitals carriing out joint training with local health authorities and community health service centers is a useful attempt.
