摘 要:目的检测雌激素受体α(estrogen receptorα,ERα)、雌激素受体β(estrogen receptorβ,ERβ)及P53在与肥胖相关子宫内膜癌患者癌组织中的表达,分析上述因子与肥胖指标的相关性,探讨ERα、ERβ及P53表达变化与肥胖诱发子宫内膜癌的相关性。方法2015年9月至2017年2月,收集石河子大学医学院第一附属医院妇产科住院患者新鲜子宫内膜及内膜癌组织150例,其中正常个体120例:根据体质指数(body mass index,BMI)分为正常体重组60例(normal control,NC,18.0 kg/m^2≤BMI≤23.9 kg/m^2)及超重及肥胖组60例(obesity,OB,BMI≥24 kg/m^2);子宫内膜癌组30例(cancer,Ca;其中子宫内膜典型增生7例,子宫内膜癌23例;18.0 kg/m^2≤BMI≤23.9 kg/m^2共8例,BMI≥24 kg/m^2,共22例)。收集患者一般资料,检测受试个体血糖、血脂及血浆游离脂肪酸(free fatty acid,FFA)水平,采用实时定量-PCR法(quantitative real-time PCR,qRT-PCR)检测子宫内膜组织中ERα、ERβ、P53的mRNA表达水平。结果OB及Ca组个体体重、BMI、腹围(abdominal circumference,WC)、血浆甘油三酯(triglyceride,TG)、血浆胆固醇(total chdestrerol,TC)、低密度脂蛋白(low density lipoprotein,LDL)、游离脂肪酸(free fasty aid,FFA)水平均显著高于NC组(P<0.05);Ca组患者癌组织中ERαmRNA表达水平及ERα/ERβ比值显著低于NC组正常内膜组织(P<0.05),P53 mRNA表达水平显著高于NC和OB组正常内膜组织(P<0.05);Ca组超重及肥胖患者癌旁组织中ERα、ERβ及P53的mRNA表达水平显著高于BMI正常的子宫内膜癌患者(P<0.05)。结论脂代谢异常及高水平FFA与肥胖诱发的子宫内膜癌相关,ERα低表达、ERα/ERβ比值降低、突变型P53高表达促进了肥胖患者子宫内膜癌的发生发展。Objective To detect the expression of estrogen receptorα(ERα),estrogen receptorβ(ERβ)and P53 in the cancer tissues of obese patients with endometrium and to analyze the correlation between the above factors and obesity indicators,and to explore the correlation between the expression changes of ERa,ERβand P53 and obesity-induced endometrial cancer.Methods From September 2015 to February 2017,150 cases of fresh endometrial and endometrial cancer tissues were collected from gynecological and obstetric inpatients in the First Affiliated Hospital of Shihezi University Medical College.120 cases of normal individuals were divided into normal weight group normal control group(normal control,NC,18.0 kg/m^2≤BMI≤23.9 kg/m^2,n=60),overweight or obesity group(obesity,OB,BMI≥24 kg/m^2,n=60)according to body mass index(BMI);there were 7 cases of atypical hyperplasia and 7 cases of endometrial cancer(cancer,Ca,n=23).General data were collected to detect the levels of blood sugar,blood lipid and plasma free fatty acid(FFA)in subjects.The expression levels of ERα,ERβand P53 in endometrial tissues were detected by real-time quantitative-PCR(qRT-PCR).Results The body weight,BMI,abdominal circumference(WC),plasma triglyceride(TG),plasma cholesterol(TC),low density lipoprotein(LDL),FFA levels in OB and Ca groups were significantly higher than those in NC group(P<0.05);the expression level of ERαand the ratio of ERα/ERβin cancer tissues of Ca group were significantly lower than those in NC group(P<0.05),and the expression level of P53 gene was significantly higher than those in NC and OB groups(P<0.05).The expression levels of ERα,ERβand P53 in adjacent tissues were significantly higher than those in endometrial cancer patients with normal BMI(P<0.05).Conclusions Abnormal lipid metabolism and high level FFA are associated with obesity-induced endometrial cancer.Low expression of ERα,lower ratio of ERα/ERβand high expression of mutant P53 promote the occurrence and development of endometrial cancer in obese patients
