摘  要:目的分析一起由副溶血性弧菌引起的食物中毒事件的致病因素,查找来源,为预防控制疫情提供科学依据,积累应急处置经验。方法采用GB 4789.7-2013《食品安全国家标准食品微生物学检验副溶血性弧菌检验》、WS 271-2007《感染性腹泻诊断标准》方法,对食物中毒的样本(患者大便/肛拭29份,厨师肛拭5份,食品及环境样本23份)进行病原菌分离,对分离的病原菌进行血清学分型鉴定,运用荧光定量PCR进行tdh、trh毒力基因检测,脉冲场凝胶电泳技术进行同源性分析。结果从57份标本中分离到29株副溶血性弧菌,27株血清型为O4:KUT型,2株副溶血性弧菌的血清型为O1:KUT。PFGE聚类分析显示,指纹图谱按100%相似度,可分为3种PFGE带型(P1、P2、P3),P1和P2带型之间相似性较强,为94%~96%,P3与P1和P2带型之间的相似性都较差,<70%。毒力基因检测,P1和P2型(27株)tdh+/trh-,P3型tdh-/trh-,tdh携带率为93.10%。结论此起食物中毒主要为O4群携带有tdh毒力基因的多种不同型别副溶血性弧菌引起,污染源主要来自鱼。Objective To analyze the pathogenic factors of food poisoning caused by Vibrio parahaemolyticus,find the source,provide scientific basis for prevention and control of the epidemic situation,and accumulate emergency treatment experience.Methods The pathogens were isolated from food poisoning samples(29 samples of stool/anal swab,5 samples of chef anal swabs,23 samples of food and environmental)using the methods in GB 4789.7-2013 Food Safety National Food Microbiological Testing Vibrio parahaemolyticus Test and WS 271-2007 Diagnostic Criteria for Infectious Diarrhea.The isolated pathogens were subjected to serological typing,and tdh,trh virulence genes were detected by fluorescence quantitative PCR,and homology analysis was performed by pulsed-field gel electrophoresis.Results Twenty nine strains of Vibrio parahaemolyticus were isolated from 57 specimens,27 strains were 04:KUT type,and 2 strains of Vibrio parahaemolyticus were 01:KUT.PFGE cluster analysis showed that the fingerprints were 100%similar and could be divided into three PFGE bands(Pl,P2,P3).The similarity between Pl and P2 bands was within 94%-96%,and the similarity between P3 and Pl,P2 was poor,less than 70%.Virulence gene detection showed that the carrying rate of Pl and P2 type(27 strains)tdh+/trh-,P3 type tdh-/trh-,tdh was 93.10%.Conclusion This food poisoning is mainly caused by a variety of different types of Vibrio parahaemolyticus carrying the tdh virulence gene in the 04 group,mainly from fish.
