摘  要:目的探讨儿科护士工作-家庭冲突、职业认同对离职意愿的影响,为降低儿科护士离职率提供一定的理论参考。方法采用整群抽样法抽取泰安市6所不同级别医院的442名儿科护士,用横断面调查法对其进行工作-家庭冲突、职业认同、离职意愿相关性的研究。结果儿科护士的工作-家庭冲突总分、职业认同总分、离职意愿总分分别为57.50±9.34,97.45±16.37和15.66±3.08;不同工作压力、聘任方式的儿科护士离职意愿差异有统计学意义(P<0.05);工作-家庭冲突对儿科护士的离职意愿具有直接正向预测作用(r=0.15,P<0.01),对职业认同有负向预测作用(r=-0.11,P<0.01),职业认同对离职意愿有负向预测作用(r=-0.37,P<0.01);工作-家庭冲突可以通过职业认同的中介效应间接影响儿科护士的离职意愿,其间接效应为0.041,中介效应占总效应的21.34%。结论儿科护士的工作-家庭冲突强、职业认同水平中等、离职意愿较强;工作压力和聘任方式是离职意愿的重要影响因素。临床医院应针对儿科工作特点采取措施,减轻工作压力,增加聘任机会,提高职业认同感,降低离职意愿,从而确保儿科护理的稳定性,提高护理质量。Objective To discuss the influence of work-family conflict and professional identity on turnover intention of pediatric nurses,so as to provide a theoretical reference for reducing the turnover rate of pediatric nurses.Methods A total of 442 pediatric nurses from 6 hospitals in Tai'an were selected by cluster sampling.A cross-sectional survey was conducted to study the correlation among work-family conflict,career identity and turnover intention.Results The total scores of work-family conflict,occupational identity and turnover intention of pediatric nurses were 57.50±9.34,97.45±16.37 and 15.66±3.08.There were significant differences in turnover intention of pediatric nurses with different job pressure and employment methods(P<0.05).Work-family conflict can directly and positively predict the turnover intention of pediatric nurses(r=0.15,P<0.01),negative prediction of career identity(r=-0.11,P<0.01),career identity has a negative predictive effect on turnover intention(r=-0.37,P<0.01).Work-family conflict can indirectly affect the turnover intention of pediatric nurses through the mediating effect of occupational identity,the indirect effect is 0.041,and the mediating effect accounts for 21.34%of the total effect.Conclusion Pediatric nurses had strong work family conflict,secondary career identity and high turnover intention.Job stress and employment style were important factors affecting turnover intention.Clinical hospitals should take measures according to the characteristics of pediatric work to reduce work pressure,increase employment opportunities,improve professional identity and reduce turnover intention,so as to ensure the stability of pediatric nursing and improve the quality of nursing.
