摘  要:目的探讨医患关系、组织支持感与医生职业承诺的相互关系,为提高医生职业承诺提出管理对策。方法采用医患关系量表、组织支持感量表和职业承诺量表等对2017年9月参加中国住院医师规范化培训高峰论坛的1973名医生进行问卷调查,运用SPSS 20.0及AMOS 17.0进行统计分析。结果医患关系与组织支持感、职业承诺呈正相关关系(r=0.436和0.540,P<0.01),组织支持感与职业承诺呈正相关关系(r=0.533,P<0.01)。结构方程分析显示,医患关系对组织支持感直接效应为0.52,组织支持感对职业承诺的直接效应为0.28,对职业承诺的直接效应为0.51;医患关系通过组织支持感间接影响职业承诺,其间接效应值为0.146,且中介效应占总效应的比例为22.2%。结论医患关系对医生职业承诺有正向作用,其中组织支持感在医患关系与职业承诺关系中起到重要中介作用,学历和年资是影响医患关系和职业承诺的显著影响因素。建议多渠道全面构建和谐医患关系,重视医生需求,提高组织支持水平,特别要加强对低学历和低年资医生的培训、组织支持和重视程度。Objective To explore the association between doctor-patient relationship,organizational support,and professional commitment among doctors,and put forward corresponding management measures to improve the professional commitment among doctors.Methods The doctor-patient relationship scale,the organizational support scale,and the professional commitment scale were conducted among 1973 doctors,who participated in the conferences of standardized training for residents on September in 2017.All statistical analyses were performed using SPSS 20.0 and AMOS 17.0 software.Results The doctor-patient relationship was positively associated with organizational support and professional commitment,respectively(r=0.436,r=0.540,P<0.01),and organizational support was positively associated with professional commitment(r=0.533,P<0.01).This structural equation analysis showed that the direct effect of doctor-patient relationship on organizational support was 0.52,and the direct effect of organizational support on professional commitment was 0.28,while the direct effect of doctor-patient relationship on professional commitment was 0.51.In addition,the doctor-patient relationship has an indirectly effect on professional commitment through organizational support,with an estimate of 0.146,and the mediating effect accounted for 22.2%for the total effect.Conclusion Doctor-patient relationship has a positive effect on professional commitment among doctors,and organizational support plays an important mediating role in the association between doctor-patient relationship and professional commitment.Educational level and work tenure are significant factors associated with doctor-patient relationship and professional commitment.It is recommended to build a harmonious doctor-patient relationship via multiple ways,and pay attention to the demands of doctors,and improve the levels of organizational support.In particular,the training,organizational support and emphasis should be provided for doctors with a low level of education and shorter of work te
