摘  要:目的通过实验室检测结果进行病原体相关性研究,分析导致引起食源性疾病的真正病因,为类似事件的实验室检验提供参考。方法 2018年12月广西某市发生1起聚餐引起的疑似食源性疾病事件。采集参与聚餐的腹泻患者肛拭标本5人份,参照GB 4789《食品微生物学检验》以及本实验室建立的快速检测方法进行病原体的检测,将分离到的病原菌进行诊断血清分型、毒力基因检测及脉冲场凝胶电泳(PFGE)分子分型等实验以分析其相关性。结果该起食源性疾病的5份标本中诺如病毒阳性3份,其中GII型2份,GI/GII混合型1份;检出致泻大肠埃希菌2份,分离到的2份标本检出2株致泻大肠埃希菌uidA和eae基因阳性,均为肠道致病性大肠埃希菌(EPEC);检出副溶血性弧菌2份,其毒力基因相同,均为tdh+、trh-、tlh+、ORF8+;脉冲场凝胶电泳分析结果显示,2株副溶血性弧菌具有100%相似度。结论该起食源性疾病事件的病因是由GI、GII型诺如病毒合并副溶血性弧菌混合感染引起。建议在处理类似事件过程中运用各种新的科学检测方法进行全局分析,才能有效地避免出现误诊和漏诊。Objective To analysis the real causes of foodborne diseases and provide a reference for laboratory testing of similar events from testing results of pathogens and the correlation analysis. Methods One suspected foodborne disease event occurred in a city of Guangxi Zhuang Autonomous Region in December 2018. Five anal swabs were collected from diarrhea patients who par-ticipated in the dinner party. The pathogens were detected according to the Food microbiology test(GB 4789) and the rapid detection method established by our laboratory. The isolated pathogenic bacteria were tested by diagnostic serum typing,virulence gene detection and pulsed field gel electrophoresis(PFGE) for molecular typing,to analyze the correlation. Results The number of three out of five fecal samples were positive for norovirus which caused by two norovirus GⅡand one norovirus GⅠ/GⅡmixed type. Two strains of Escherichia coli were isolated from two samples with genotypes of uidA and eae respectively,both were Enteropathogenic Escherichia coli(EPEC). Vibrio parahaemolyticus were isolated from two samples with the same characteristic virulence genotype of tdh+,trh-,tlh+ and ORF8+. The results of PFGE showed that the two strains of Vibrio parahaemolyticus had100% similarity. Conclusion This foodborne disease event is caused by the mixed infection of norovirus genotype GI and GII combined with Vibrio parahaemolyticus. In order to avoid misdiagnosis and missed diagnosis effectively,it is suggested that all kinds of new scientific detection methods should be adopted to analyze the comprehensive situation in the process of handling similar events.
