摘  要:2020年冬春之交的新型冠状病毒肺炎疫情中,基层全科医生全力参与流行病学调查、患者的诊断和治疗、公众教育及社区抗疫指导,使人们再次聚焦全科医生及时、快速、长期、持续的医疗照顾。但此过程中也暴露出推进适当转诊、分诊和维持有效的分级诊疗体系依然挑战重重。笔者认为,新冠肺炎之役充分体现了全科医学在分级诊疗和公共卫生中的重要作用,全科医生的职业素养和专业能力至关重要,应将更多资源投入到全科医学的毕业后教育,在全国范围内广泛培养合格的全科医生。由此引发相关的思考和建议:(1)医学人文教育应与专业教育相结合,重视医学史的系统教育;(2)加强全科医生基层医疗服务能力的培养;(3)优化培训方式,适当延长毕业后医学教育的社区实践时间。During the novel coronavirus(COVID-19)pneumonia pandemic in the spring of 2019,general practitioners(GPs)working in the community devoted themselves to epidemiological investigations,diagnosis and treatment of patients,public education and leading the community against the pandemic.The COVID-19 pandemic refocused public attention on the“first in last out”role of the GPs.However,there remained challenges to appropriate referral,triage and maintenance of an effective tiered health care system.It is believed that the battle against COVID-19 has highlighted the irreplaceable role of general practice in tiered health care and public health.The professionalism and competence of the GPs are of vital importance.More resources should be devoted to post-graduate education in general practice so as to train a larger number of qualified GPs across the country.Ways to improve post-graduate education of GPs include integrating medical humanistic education into medical education and attaching great importance to education on the history of medical sciences,reinforcing community-based medical and health service and optimizing training methods and appropriately extending the length of service in a community in standardized training for GPs.
