基于疫情监测的住院医师规范化培训管理实践

摘　　要：本研究通过运用新型冠状病毒肺炎疫情数据和重要政策,探索疫情下的住院医师规范化培训管理模式。研究将武汉市疫情分为早期、上升期、下降期和恢复期四个时期,从临床培训管理、学员安全防护、学员疫情管理、学员生活保障和学员舆情管理五个方面进行疫情期间住培管理。研究中疫情期间管理实践主要包括:(1)住培学员坚守在抗击新冠肺炎工作岗位,为有效阻击新冠肺炎疫情做出了积极贡献;(2)职能部门根据不同时期的疫情形势,先后发布学员安全防控和培训管理方案等文件12个,有效保障了疫情期间的住培工作稳定开展;(3)及时有效处理网络舆情危机3件次,有效维护疫情期间的学员思想和舆情稳定;(4)在疫情恢复期率先启动2020年住培招收宣传工作,积极引导全省逐步恢复住培工作。培训基地在面对重大突发公共卫生事件时,住培管理主要遵循以下三个指导原则:(1)培训基地需尽最大努力保障住培学员安全;(2)住培学员既是培训对象,也是临床医师的重要组成部分,应坚决捍卫人民群众生命安全和身体健康,在抗击疫情中练就过硬本领;(3)培训基地需综合考虑学员安全防护和培训计划等各方面要求,根据疫情防控指挥部统一部署制定具体管理方案。The epidemic data on and important guidelines for COVID-19 were used to explore management modes of residency training amid the epidemic.The COVID-19 epidemic in Wuhan was divided into four periods:the early period,ascending period,descending period,and recovery period.Residency training during the epidemic centered on clinical training management,safety protection of residents,epidemic control of residents,life support of residents and opinion control of residents.The main management practices included:(1)Residents stayed on their jobs to fight against novel coronavirus pneumonia,and made a positive contribution.(2)Twelve documents about residents’safe prevention and training management were issued in different periods,which ensured the stability of residency during the epidemic.(3)Three public opinion crises among residents were effectively dealt with so that residents’thoughts and opinions remained unchanged during the epidemic.(4)Advertising for resident recruitment for 2020 started at the beginning of the recovery period,thus leading the province to gradually resume residency training.Residency training amid the epidemic proceeded according to the three guidelines:(1)Training institutions needed to do their best to ensure the safety of residents.(2)Residents were not only trainees,but also an important part of clinicians who were committed to human lives and health while improving their skills against the epidemic.(3)Training institutions needed to take into serious consideration the requirements of resident safety protection and training plans,and to develop specific management plans based on the instructions of the Epidemic Prevention and Control Headquarters.
