全自动毛细管电泳技术在筛查地中海贫血中的临床诊断价值

摘  要:目的探讨在进行地中海贫血筛查中全自动毛细管电泳技术应用可行性。方法选择贺州市妇幼保健院2019年11月至2020年3月收治的3500例地中海贫血筛查患者作为实验对象,分别利用全自动毛细管电泳技术以及琼脂糖凝胶电泳技术进行诊断,以PCR基因诊断作为参考依据,比较两种筛查技术检出率。结果PCR基因诊断地中海贫血患者共995例,其中405例α-地中海贫血,毛细管电泳技术确诊370例(37.19%),凝胶电泳技术确诊270例(66.67%);480例β-地中海贫血,毛细管电泳技术确诊452例(94.17%),凝胶电泳技术确诊340例(70.83%);110例HbH病患者,毛细管电泳技术确诊110例(100.00%),凝胶电泳技术确诊107例(97.27%)。全自动毛细管电泳技术组α-地中海贫血及β-地中海贫血检出率均明显高于琼脂糖凝胶电泳技术组(P<0.05);HbH病检出率同琼脂糖凝胶电泳技术组比较,差异无统计学意义(P>0.05)。地中海贫血患者MCH以及MCV水平与HbH患者比较,差异有统计学意义(P<0.05)。结论全自动毛细管电泳技术可显著提升α-地中海贫血及β-地中海贫血检出率,最终实现地中海贫血疾病的早期确诊、治疗以及有效预后。Objective To explore the feasibility of automatic capillary electrophoresis in thalassemia screening.Methods From November 2019 to March 2020,3500 patients with thalassemia were screened in Hezhou Maternal and Child Health Hospital using automatic capillary electrophoresis(ACE)and agarose gel electrophoresis(AGE),respectively.PCR gene diagnosis was used as a reference to compare the detection rates of the two screening techniques.Results There were 995 cases of thalassemia diagnosed by PCR gene,including 405 cases ofα-thalassemia,370 cases(37.19%)confirmed by ACE and 270 cases(66.67%)confirmed by AGE,as well as 480 cases ofβ-thalassaemia,452 cases(94.17%)confirmed by ACE and 340 cases(70.83%)confirmed by AGE.Of the 110 HbH patients,110 cases(100.00%)were confirmed by ACE and 107(97.27%)by AGE.The detection rates ofα-andβ-thalassemia in the ACE group were significantly higher than those in the AGE group(P<0.05).The detection rate of HbH was not significantly different from that of AGE group(P>0.05).Conclusion ACE can significantly improve the detection rate ofα-andβ-thalassemia,and ultimately realize the early diagnosis,treatment and effective prognosis of thalassemia disease..
