摘  要:目的通过对一起输入型食源性疾病的病原学鉴定和分子溯源分析,为科学指导临床诊断治疗及流行病学调查工作提供参考。方法收集20例腹泻病患者粪便标本,分别采用Filmarray快速鉴定和分离培养的方法,对分离到的菌株进行血清分型、脉冲场凝胶电泳(PFGE)分型、全基因组测序(WGS),通过基因序列比对对病原菌的毒力岛、毒力基因、耐药基因和质粒携带情况进行分析,通过多位点序列分型(MLST)和单核苷酸多态性(SNP)对菌株进行溯源。结果从2位患者粪便标本中分离培养得到的肠炎沙门菌PFGE型别一致,与本地沙门氏菌PFGE图谱有显著区别,分离株的毒力岛、毒力基因、耐药基因携带情况各不同,但都拥有一处非同义突变(gyrA基因),可能影响萘啶酮酸、环丙沙星的耐药性, MLST均为ST11型,SNP聚类结果显示与泰国临床分离株较为接近。结论加强入境旅客的健康监测,进一步做好输入性传染病防控工作,建立健全输入型疾病预警机制,尤其注意防范耐药菌输入风险的发生。Objective Through the pathogen identification and molecular traceability analysis of an imported foodborne disease,it provided reference for the scientific guidance of clinical diagnosis and epidemiological investigation.Methods 20 cases of diarrhea patients stool specimens were collected.Filmarray rapid identification and culture methods were used respectively for the specimens.Serotyping,pulsed field gel electrophoresis(PFGE)typing and whole genome sequencing(WGS)were used for isolated strains.Virulence island,virulence genes,resistance gene and plasmid carrying were analyzed by sequence alignment.Multiple loci sequence classification(MLST)and single nucleotide polymorphisms(SNPS)were used for strains molecular tracing.Results The PFGE type of Salmonella enteritis isolated from feces samples of 2 patients was consistent.It was significantly different from the local Salmonella strains.The virulence island,virulence gene and drug resistance gene of the isolates were different,but there was a non-synonymous mutation(gene gyrA)in both of them.The isolated strains were both ST11.The SNP clustering results showed that the strains were similar to those of Thailand clinical isolates.Conclusion It is important to perform health monitoring of passengers from aboard.To improve the prevention and control of import diseases,it is important to establish an early warning mechanism.In particular,we should pay more attention to the risk of drug-resistant bacteria.
