一起由O3:K6血清型副溶血性弧菌引起的食物中毒暴发调查

摘　　要：目的 查明江门市一起食物中毒发生的原因,为预防类似事件发生提供依据。方法 接报后通过查看病例周边医院的就诊记录以及病例所在公司的员工缺勤登记表进行病例搜索;采用统一问卷调查员工进食情况,采用队列研究寻找危险因素,采集病例肛拭子和剩余食品等进行细菌分离培养、检验和脉冲场凝胶电泳分子分型。结果 病例均为该公司员工,罹患率为42.7%。流行曲线提示为点源暴发。流行病学调查证据显示8月11日午餐是危险餐次,回顾性队列研究显示烧鸭是危险食物(RR=116.211,95%CI:14.943~903.743)。从病例的肛拭子和剩余食品中检出O3:K6血清型副溶血性弧菌,脉冲场凝胶电泳分子分型提示高度同源。结论 该起事件为一起由O3:K6血清型副溶血性弧菌引起的食物中毒,可能为烧鸭受副溶血性弧菌污染,食用前加热不彻底所引起。Objective To find out the cause of a food poisoning in Jiangmen city and provide basis for the prevention and control of similar incidents. Methods After receiving the report, we searched for the related cases by checking the medical records of the hospitals around the reported cases and the absence registration form of the employees of the company where the reported cases worked. A uniform questionnaire was used to investigate employees’ diet, cohort study was used to find risk factors, and anal swabs and residual food were collected for bacterial isolation, culture, testing and molecular typing by pulse field gel electrophoresis. Results All cases were employees of the company, with a morbidity rate of 42.7%. The epidemic curve indicated a point source outbreak. Epidemiological investigation showed that lunch on August 11 was a dangerous meal, and retrospective cohort study showed that roast duck was a dangerous food(RR=116.211, 95%CI:14.943-903.743).The vibrio parahaemolyticus serotype O3:K6 was detected from the anal swabs of the cases and food residues, and the typing by pulse field gel electrophoresis suggested a high degree of homology. Conclusion This incident was a food poisoning caused by vibrio parahaemolyticus serotype O3:K6, which might be caused by the cooked duck being contaminated by vibrio parahaemolyticus and not thoroughly heated before eating.
