摘 要:目的了解山东省住院医师胜任力水平并分析其影响因素,从而提出有针对性的意见和建议,为提高山东省住院医师胜任力水平和培训质量提供有效帮助和有益参考。方法利用非参数检验、Kruskal-Wallis H检验等统计学方法对2019年山东省34家培训基地的1952名住院医师胜任力水平及其影响因素进行分析。结果本研究共回收问卷1963份,其中有效问卷1952份,有效率为99.4%。山东省8家培训基地住院医师胜任力总体得分中位数为40.00分(满分45.00分)。五大维度满分均为9.00分,其中医学知识得分中位数为8.00分,临床操作能力得分中位数为7.67分,临床思维得分中位数为8.00分,沟通技能得分中位数为8.00分,职业素养得分中位数为9.00分。五大维度中,临床操作能力得分较低(P<0.05)。不同学历、培训身份属性、通过执业医师资格考试时间、单位人就业单位性质等研究因素均与住院医师胜任力水平具有相关性(P<0.05)。结论建议以住院医师总体胜任力为导向,提高住院医师培训质量,立足不同影响因素,不断提高住院医师在各维度的胜任力水平,为分级诊疗提供人才保障。Objective To understand the competency level of residents in Shandong Province and analyze the influencing factors of its compe-tence,so as to provide targeted opinions and suggestions,and provide effective help and useful reference for improving the competency level and training quality of residents in Shandong Province.Methods Using statistical methods such as nonparametric test and Kruskal-Wallis H test to ana-lyze the competency level and influencing factors of 1952 residents in 34 training bases in Shandong Province in 2019.Results 1963 question-naires were collected in this study,of which 1952 were valid questionnaires,with an effective rate of 99.4%.The median overall competency score of residents in eight training bases in Shandong province was 40.00(over 45.00 scores).The five points have full score of 9.00 scores,with median medical knowledge score of 8.00 scores,median clinical ability score of 7.67 scores,median clinical thinking score of 8.00 scores,and median com-munication skills score of 8.00 scores,the median professional literacy score was 9.00 scores.Among the five dimensions,the clinical ability score was low(P<0.05).Research factors such as different academic qualifications,training status attributes,time to pass the qualification examination for practicing doctors,and the nature of the employment unit were the influencing factors of resident's competence level(P<0.05).Conclusion It is suggested that the overall competency of residents should be taken as the guidance,the training quality of residents should be improved,and the competency level of residents should be continuously improved based on different influencing factors so as to provide qualified personnel for grading diagnosis and treatment.
