摘 要:目的 评价目标教学法结合迷你临床演练评估在消化内科专业住院医师规范化培训中的作用,提高消化专科住院医师规范化培训的教学水平。方法 对2014年1月1日-2019年1月1日间进入消化内科进行三年住院医师规范化培训的医生132人随机分为传统教学方法组(n=65)和目标教学方法组(n=67),在住院医师进入消化内科培训前后随机进行两次Mini-CEX测评,并在培训后进行问卷调查。结果 两组的问卷调查结果整体比较显示目标教学组的“非常满意”比例明显高于传统组,“不满意”比例低于传统组(P=0.006,P<0.05)。目标教学组和传统组在培训后每项考评项目成绩均较培训前有明显升高;但是,目标教学组与传统组分别比较每个考核项目的培训后成绩,发现除“宣教咨询”项目外(P=0.100),其他考评项目目标教学组的成绩均高于传统组。结论 在消化内科专业住院医师规范化培训中,应用目标教学法结合Mini-CEX可有效提高住院医师的培训成绩,提高消化内科临床教学质量,并获得住院医师的认可。Objectives To evaluate the effect of target teaching method combined with mini clinical exercise assessment(Mini-CEX) in the standardized training of residents in gastroenterology department.Methods A total of 132 residents from January 1 st 2014 to January 1 st 2019 were randomly divided into the traditional teaching method group(n = 65) and the target teaching method group(n=67).The Mini-CEX was randomly evaluated twice at the beginning of the training and after of the training,and the questionnaire survey was conducted after the training.Results The overall comparison of the two groups showed that the "very satisfied" rate of the target teaching group was significantly higher than that of the traditional group,and the "dissatisfied" rate was lower than that of the traditional group(P=0.006,P<0.05).After training,the scores of each evaluation item in the target teaching group and the traditional group were significantly higher than those before training.However,it was found that the scores of the target teaching group were higher than those in the traditional group except for the "propaganda and consultation" item(P =0.100) comparing the scores of each evaluation item in the target teaching group and the traditional group.Conclusions In the standardized training of residents in gastroenterology,the application of target teaching method combined with Mini-CEX can effectively improve the training performance of residents,improve the quality of clinical teaching in gastroenterology,and obtain the recognition of residents.
