摘 要:目的 对一起企业(S公司)食堂食源性疾病暴发事件进行溯源。方法 按照相关技术指南开展现场流行病学调查、食品卫生学调查和实验室检验,运用EpiInfo 7和SPSS 18.0软件进行统计分析,并采用脉冲场凝胶电泳(PFGE)进行同源性分析。结果 本次事件累计发现病例66例,罹患率6.59%,临床表现以腹泻、腹痛、发热、乏力为主,潜伏期中位数12.75 h,可疑餐次为2020年7月4日晚餐(χ^(2)=43.847,P<0.001);病例对照研究显示,可疑食品为番茄菜花炒鸡蛋(OR=18.667,95%CI:6.877~50.665)和白菜油皮(OR=6.600,95%CI:2.300~18.941);患者便样和7月4日晚餐菜品留样中检出19份肠炎沙门氏菌,通过PFGE分析为2个型别,株菌间相似度为60.4%~100.0%。结论 此次暴发是S公司食堂2020年7月4日晚餐加工过程中肠炎沙门氏菌污染所致。Objective To trace the source of a foodborne disease outbreak in a corporate canteen.Methods According to the relevant technology guide,epidemiological investigation,hygienic investigation and laboratory testing were performed.Epilnfo 7 and SPSS 18.0 were used to conduct the statistical analysis,and pulsed field gel electrophoresis(PFGE)was used to evaluate the strain homology.Results A total of 66 cases were reported,and the attack rate was 6.59%.The main clinical symptoms were diarrhea,abdominal pain,fever and fatigue;the median incubation period was 12.75 hours.The exposed sources were the dinner on July 4,2020(χ^(2)=43.847,P<0.001).The case-control study results showed that the suspicious food was scrambled eggs with tomato and cauliflower(OR=18.667,95%Cl:6.877-50.665),Stir-fried tofu skin with cabbage(0R=6.600,95%Cl:2.300-18.941).Salmonella enteritidis was detected in 19 samples of patients'stool and food samples for the dinner on July 4,which were classified into 2 types by PFCE,and the similarity between strains was 60.4%-100.0%.Conclusions This outbreak was caused by cross contamination of Salmonella enteritidis during the processing of the dinner on July 4,2020 in the canteen.
