摘 要:目的 比较传统考试与客观结构化临床考试(OSCE)在护理本科生毕业前技能考核中的应用效果。方法 采用前瞻性研究,从我校2017级护理本科生中选取在直属医院实习的128人作为实验组,在附属西南医院实习的120人作为对照组。实验组采用OSCE模式,对照组采用传统考试模式,从技能考核、专科案例、考站整体、综合成绩四个维度对两组成绩进行分析,并做考试方案信度、结构效度分析及两组对考试评价的比较。结果 除儿科案例成绩实验组低于对照组外,四个维度上两组成绩比较均无显著差异(P>0.05)。OSCE信度和结构效度优于传统考试。实验组对考试认同率高于对照组(P<0.05);考试中紧张情绪出现率明显低于对照组,考试整体满意度明显高于对照组(P<0.001)。结论 在护理本科生毕业前技能考核中,OSCE能客观、公平、有效、可靠地评价考生的临床综合能力,考站设计及单站分数权重分配合理,应用效果优于传统考试,考生应试压力低于传统考试,值得推广。Objective To compare the application effect of traditional examination and Objective Structured Clinical Examination(OSCE)in the skill examination of nursing undergraduates before graduation.Methods In this prospective study,128 nursing undergraduates in grade 2017 who were doing the internship at the affiliated hospital of our university were selected as the experimental group,and 120 who were doing the internship at the Affiliated Southwest Hospital were selected as the control group.The experimental group adopted OSCE mode,and the control group adopted traditional examination mode.The exam result of the two groups was analyzed from four dimensions,including single station,specialized subject case,the whole station and comprehensive score.Reliability and structural validity of examination scheme was analyzed,and evaluation of examination from the two groups was compared,too.Result There was no significant difference between the two groups(P<0.05)in the four dimensions,except that the score of pediatric case in the experimental group was lower than that in the control group.OSCE is superior than traditional examination in reliability and structure validity.Acceptance rate of exam in the experimental group was higher than that in the control group(P<0.05).Occurrence rate of tension during exam in the experimental group was significantly lower than that in the control group,and overall satisfaction with exam in the experimental group was significantly higher than that in the control group(P<0.001).Conclusion In the skill examination of nursing undergraduates before graduation,OSCE which can evaluate candidates'clinical comprehensive ability objectively,fairly,effectively and reliably,and of which exam station design and single-station's score weight distribution are reasonable,is better than traditional examination in application effect.Test pressure of candidates in OSCE is lower than that in traditional examination,so it is worth promoting.
