改良版精神科住院医师规范化培训结业实践技能考核方案在湖北省的应用

摘　　要：目的 探讨改良版精神科住院医师规范化培训(简称住培)结业实践技能考核方案在湖北省的应用效果,为精神科结业实践技能考核持续改进提供科学依据。方法 以2021年和2022年参加湖北省精神科住培结业考核的74名住院医师为研究对象,对实践技能考核的具体实施情况、考核成绩和考核反馈进行回顾分析和总结。结果 改良版精神科住培结业考核在湖北省的应用效果良好,住院医师考核合格率为100%,总成绩平均85.22分;不同学历、不同住培基地、不同来源考生成绩差异无统计学意义(P> 0.05);方差分析提示临床沟通得分显著高于临床技能、临床思维,神经系统检查得分显著高于临床思维,差异均有统计学意义(均P<0.05)。考官及考生对考核内容、难度、形式及时间的合理性基本为非常同意,其次为同意;认可度较高。结论 目前湖北省内精神科住培质量基本实现同质化,在实践技能培训中临床思维为相对薄弱环节;改良版精神科住培结业实践技能考核的认可度较高,能够公平、客观地评价住院医师的实践技能水平,有利于精神科住培质量的持续提升。Objective To explore the application effectiveness of an improved assessment of clinical practice skills during standardized training for resident psychiatrists in Hubei Province and to provide scientific evidence for the continuous improvement of the assessment.Methods A retrospective analysis was conducted on 74 psychiatric residents who participated in the standardized training and assessment for psychiatric residents in Hubei Province in 2021 and 2022.The implementation of the practical skills assessment,assessment results and feedback were reviewed and analyzed.Results The application of improved assessment of clinical practice skills during standardized training for resident psychiatrists in Hubei Province was effective,with a 100%pass rate and an average total score of 85.22.There were no significant differences in scores among different educational backgrounds,training bases and sources of trainees(P>0.05).The analysis of variance showed the score for clinical communication was significantly higher than those for clinical skills and clinical thinking,and the score for neurological examination was significantly higher than that for clinical thinking with statistically significant difference(all P<0.05).The examiners and examinees generally agreed that the assessment content,difficulty,format,and time were reasonable,with high approval rates.ConclusionThe quality of psychiatry residency training in Hubei Province has achieved homogenization.Clinical thinking is a relatively weak aspect of training in practical skills.The improved assessment of clinical practice skills for resident psychiatrists has a high approval rate and can objectively evaluate their level,which is conducive to the continuous improvement of psychiatry residency training quality.
