摘 要:目的 分析比较两种不同的内科住院医师在感染科培训模式,探讨提升内科住院医师感染性疾病临床实践能力的适宜培训方案。方法 以复旦大学附属医院完成感染科轮转的住院医师为调研对象,采用方便抽样法,收集住院医师对培训的评价和满意度情况,以培训前后住院医师的自评得分差值来衡量感染性疾病临床实践能力提升幅度,采用秩和检验或多元线性回归统计分析两种不同的感染科培训模式的效果。结果 复旦大学附属医院内科住院医师对感染科轮转整体评价较好;住院医师感染性疾病临床实践能力均得到了提升;感染性疾病临床实践能力提升幅度与管理的病种数量呈显著正相关,并在不同模式或不同级别的医院之间存在显著差异。结论 感染科培训应确保病种数量。感染科轮转质量与医院感染性疾病相关的医教研管理综合实力相关。医学院校应发挥统筹整合优质教学资源的作用,夯实附属医院教学主阵地,推进住院医师同质化培养。Objective To analyze and compare two different training modes of internal medicine residents in infection department,and to explore the appropriate training plan to improve the clinical practice ability of internal medicine residents in infectious diseases.Methods Residents who completed the training of infection department in the affiliated hospitals of Fudan University were selected as the research subjects.The convenient sampling method was adopted to collect the evaluation and satisfaction of the residents on the training.The improvement in clinical practice ability of infectious diseases was measured by the difference of the self-rated score of the residents before and after training.We used rank sum test or multiple linear regression statistics to analyze the effect of two different training rotation modes in infection department.Results Resident physicians of affiliated hospitals of Fudan University had a good evaluation on the rotation of infection department.The clinical practice ability of residents in infectious diseases was improved.The improvement in clinical practice of infectious diseases was positively correlated with the number of managed diseases,and significant differences were found among affiliated hospitals of different modes or levels.Conclusions Training of infection department should ensure the number of diseases managed by residents.The quality of rotation in infection department is related to the comprehensive strength of hospitals in medicine,teaching,research and management.Medical colleges and universities should play the role of coordinating and integrating high-quality teaching resources,tamping the main teaching position of affiliated hospitals,and promoting the homogeneous development of resident physicians.
