摘 要:目的 探讨基于真实病例的Seminar教学模式在消化内科轮转住院医师培训中的应用效果。方法 选取2018至2021年在上海交通大学医学院附属新华医院消化内科81名住院医师为研究对象,分为观察组(41人)和对照组(40人)。观察组采取基于真实病例的Seminar教学模式,对照组采取传统教学模式。培训结束后比较两组住院医师的考试成绩和临床综合能力,通过问卷对教学效果进行评价。结果 观察组理论考试和病例考核得分分别是(84.53±6.20)分和(83.22±5.58)分,高于对照组(81.02±5.30)分和(79.78±4.48)分,差异有统计学意义(均P<0.05)。观察组临床综合能力评估得分(4.36±0.39)分明显高于对照组(3.90±0.29)分,差异有统计学意义(P<0.001)。问卷显示此教学模式有助于住院医师提高学习兴趣、提升自学能力、拓展能力和系统思维能力(P<0.05),85.4%的观察组住院医师对此教学模式表示满意(P<0.05)。结论 基于真实病例的Seminar教学模式有助于提高住院医师理论知识和临床综合能力,培养住院医师的临床思维。Objective To explore the application effect of the seminar teaching model based on real cases in the standardized residency training in the department of gastroenterology.Methods A total of 81 residents from 2018 to 2021 were selected from the Department of Gastroenterology,Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine,of which 41 belonged to the observation group(seminar teaching model based on real cases)and 40 belonged to the control group(traditional teaching model).After teaching,the scores of the tests and clinical abilities of the two groups were compared,and the teaching effect was evaluated through a questionnaire survey.Results The scores of theory and cases tests in the experimental group were(84.53±6.20)and(83.22±5.58),respectively,which were higher than those in the control group[(81.02±5.30)and(79.78±4.48),respectively ] with statistical significance(all P<0.05).The total score of clinical comprehensive ability evaluation in the experimental group(4.36±0.39)was significantly higher than that in the control group(3.90±0.29),with statistically significant difference(P<0.001).The survey results showed that this teaching model was helpful to improve learning interest,self-learning ability,expansion ability and systematic thinking ability(P<0.05).85.4%of the residents in the experimental group were satisfied with this teaching model(P<0.05).Conclusions The seminar teaching model based on real cases is helpful to improve the level of basic theory and clinical knowledge,and cultivate the clinical thinking ability of the residents.
