摘 要:目的探讨利用国际顶级医学学术期刊及其官网资源对高年资住培医生临床思维培训的应用效果。方法2021年8—9月,随机选择西安交通大学第二附属医院15名三年级住院医师规范化培训(简称“住培”)医生,对其进行每周1节临床病案推理及临床思维培训课。该课程由8名西安交通大学第二附属医院急诊科授课老师授课。8名授课教师随机分为对照组(n=4)和试验组(n=4),对照组临床教学病例来源于本住培基地临床病例,试验组临床教学病例来源于国际顶级医学学术期刊报道或其官网有关医学教育内容。2名评委随堂听课。每堂课程结束后对住培医生、评委、授课教师进行问卷调查,对问卷结果进行分析。结果在病案新颖性、认知启发性、临床思维提升、科研启发、临床病案质量、学员随堂临床思维发散程度及病案对授课教师自我认知的提高方面,试验组均优于对照组,差异有统计学意义(P<0.05)。结论利用国际顶级医学学术期刊及其官网资源对高年资住培医生进行临床思维培训,能够提高住培教学质量,使得教学内容国际化,达到教学相长的效果。Objective To explore the effect of using the resources of international medical academic journals or their official websites to improve clinical thinking ability in senior doctors of standardized training.Methods From August to September 2021,a total of 15 third year resident doctors in the Second Affiliated Hospital of Xi'an Jiaotong University were randomly selected for standardized training(referred to as"resident training"),and they were given one clinical case reasoning and clinical thinking training class per week.This course was taught by 8 teachers from the emergency department of the Second Affiliated Hospital of Xi'an Jiaotong University.a total of 8 teaching teachers were randomly divided into a control group(n=4)and an experimental group(n=4).The clinical teaching cases in the control group were obtained from clinical cases in the residential training base,while the clinical teaching cases in the experimental group were obtained from reports from top international medical academic journals or relevant medical education content on their official websites.Two judges were attend the class in class.After each course,provided guidance to resident training doctors,judges,and instructors.Results The test group was better than the control group in the novelty of cases,cognitive inspiration,improvement of clinical thinking,scientific research inspiration,quality of clinical cases,divergence of clinical thinking and teachers'self-improvement(P<0.05).Conclusion Using the resources of international medical academic journals or their official websites to train senior doctors of standardized training in clinical thinking course can improve the quality of teaching,internationalize the teaching content and achieve teachers'self-improvement.
