摘 要:目的了解秦皇岛市市售食品、食源性疾病主动监测及食源性疾病暴发事件中检出的副溶血性弧菌的毒力基因携带情况和耐药性,为由该菌引起的食源性疾病的防治提供科学依据。方法对2017—2021年检出的96株副溶血性弧菌进行不耐热性溶血毒素(thermolabile hemolysin,TLH)的基因tlh、热稳定直接溶血毒素(thermostable direct hemolysin,TDH)的基因tdh及热稳定直接溶血相关毒素(thermostable direct hemolysin-related hemolysin,TRH)的基因trh进行测定和药敏实验。结果市售食品中检出的副溶血性弧菌仅携带tlh毒力基因,食源性疾病主动监测检出的副溶血性弧菌均携带tlh和tdh毒力基因,食源性疾病暴发事件中检出的副溶血性弧菌除携带tlh和tdh毒力基因外,还携带trh毒力基因;副溶血性弧菌耐药性实验结果显示,其对头孢唑林的耐药率为81.25%,对氨苄西林的耐药率为25.00%,对头孢他啶的耐药率为10.42%,对其他抗生素的耐药率为0%~10.00%;多重耐药菌株的构成比为10.42%。结论秦皇岛市不同来源的副溶血性弧菌,其携带的毒力基因呈现多样性,对不同抗生素呈现不同的耐药性,临床治疗应注意抗生素的合理应用,防止耐药菌株的出现。Objective To understand the virulence gene carrying and drug resistance of Vibrio parahaemolyticus detected in market-sold food,active surveillance of foodborne diseases,and outbreaks of foodborne diseases in Qinhuangdao City,and to provide a scientific basis for the prevention and control of foodborne diseases caused by V.parahaemolyticus.Methods The genes tlh for ther⁃molabile hemolysin(TLH),tdh for thermostable direct hemolysin(TDH),and trh for thermostable direct hemolysin-related hemolysin(TRH)were determined and drug sensitivity experiments were performed on 96 Vibrio parahaemolyticus detected from 2017 to 2021.Results Vibrio parahaemolyticus detected in market-sold food only carried the tlh virulence gene,and V.parahaemolyticus detected in active surveillance of foodborne diseases all carried tlh and tdh virulence genes,while V.parahaemolyticus detected in foodborne disease outbreaks carried trh virulence gene in addition to tlh and tdh virulence genes.The drug resistance test results of V.parahaemolyticus showed that its drug resistance rates of cefazolin,ampicillin,and ceftazidime were 81.25%,25.00%,and 10.42%,respectively.The drug resistance rates of other antibiotics were 0%to 10.00%.The proportion of multidrug-resistant strains was 10.42%.Conclusion Vibrio parahaemolyticus from different sources in Qinhuangdao City carries a variety of virulence genes and showes different resistance to different antibiotics.Clinical treatment should pay attention to the rational application of antibiotics to prevent the emergence of drug-resistant strains.
