摘  要:目的探讨DM患者HDL-C水平与膳食脂肪摄入情况的关系。方法选取2013~2018年美国国家健康与营养调查参与者29400名,纳入≥20岁DM患者1971例,根据HDL-C三分位数分为低HDL-C组(0.47~1.01 mmol/L,n=579)、中HDL-C组(1.03~1.29 mmol/L,n=698)和高HDL-C组(1.32~3.85 mmol/L,n=694),比较各组人口学特征、糖脂和膳食能量、脂肪、脂肪酸日摄入差异。结果低、中、高HDL-C组多不饱和脂肪酸占脂肪百分比依次升高(P<0.05),FPG、HbA1c、TG、膳食脂肪、饱和脂肪酸(SFA)、单不饱和脂肪酸、SFA占脂肪百分比依次降低(P<0.05)。结论DM患者高HDL-C水平糖脂控制更好,膳食脂肪、脂肪酸摄入量更低,脂肪酸结构更合理。Objective To explore the relationship between different high-density lipoprotein cholesterol(HDL-C)levels and dietary fat intake in patients with diabetes mellitus(DM).Methods The data of 29400 participants in the National Health and Nutrition Survey(NHANES)from 2013 to 2018 were screened.1971 participants with DM≥20 years were included.According to the tertiles of HDL-C,they were divided into low HDL-C group(0.47~1.01 mmol/L,n=579),medium HDL-C group(1.03~1.29mmol/L,n=698)and high HDL-C group(1.32~3.85 mmol/L,n=694).The demography characteristics,blood glucose,blood lipid and daily dietary energy,fat and fatty acid intake were compared among the three groups.Results The levels of FPG,HbA1c,TG,daily dietary fat,MUFA,SFA and the percentage of SFA in total fat in the low,medium and high HDL-C groups were decreased sequentially(P<0.05),while the percentage of PUFA in fat was increased in turn(P<0.05).Conclusion DM patients with high HDL-C level have better control of blood glucose and lipids,lower daily intake of dietary fat and fatty acids,but more reasonable fatty acid structure.
