摘  要:目的探讨分析基于专家示范的高仿真模拟教学对高职院校护生临床判断能力的影响。方法随机将护理三年制2个班的87名护生分为试验组与对照组,试验组43名,对照组44名。试验组采用基于专家示范的高仿真模拟教学方法,对照组采用常规高仿真模拟教学方法。采用美国护理联盟研发的模拟教学满意度、模拟教学自我效能感量表和拉萨特临床判断能力量表,从柯氏评估模型多层次评价基于专家示范的高仿真模拟教学在护理教育中的应用。结果试验组模拟教学满意度和自我效能感得分均略高于对照组,差异无统计学意义(P>0.05)。教师评价中试验组学生Lasater临床判断能力量表中评估观察、分析决断、实施决策3个维度和总分均高于对照组,差异有统计学意义(P<0.05)。而2组学生自我评价中临床判断量表得分比较差异无统计学意义(P>0.05)。结论基于专家示范的高仿真模拟教学,符合学生效仿榜样的潜意识需求,有效提升提高学生的临床评估、推理和决策能力。建议进一步护教协同,开发丰富的专家示范联合有声思维的信息化资源,帮助学生洞察专家在解决各种临床问题时的推理策略和思维过程。Objective To explore the effect of expert demonstration-based high-fidelity simulation on clinical judgment of nursing students in higher vocational college.Methods A total of 873-year program nursing students were randomly divided into experimental group with expert demonstration-based high-fidelity simulation(n=43)and control group receiving conventional high-fidelity simulation(n=44).Student Satisfaction and Self-Confidence in Simulation Learning developed by the National League for Nursing and the Lasater Clinical Judgment Rubric(LCJR)were used to evaluate the effect.Results Satisfaction and self-confidence score in experimental group was slightly higher,showing no statistical significance(P>0.05).In expert rating,the total score of LCJR and the score of assessment and observation,analysis and decision-making and implementation in experimental group were higher(P<0.05),but in student self-assessment,the score of LCJR indicated no statistical significance(P>0.05).Conclusion Expert demonstration-based high-fidelity simulation,catering to students’subconscious need for role modeling,effectively enhances students’clinical assessment,reasoning,and decision-making abilities.It is recommended to further promote collaborative nursing education and develop rich expert-demonstration combined with audio-enhanced thinking information resources to assist students in understanding the reasoning strategies and thought processes experts employ when solving various clinical problems.
