混合式教学模式在中医住院医师规范化培训临床思维培养中的探索与研究

摘　　要：中医住院医师规范化培训是医教协同深化中医学人才培养改革的关键环节,是加强中医临床人才队伍建设的有效途径。中医经典是中医理论的源头,是培养中医思维的最佳教材与创新能力的不竭源泉。在中医住院医师规范化培训教育中要不断提高医生的中医自信,重视中医基础理论知识、中医临床辨证思维及诊疗思路的培养,这样才能培养出合格的中医住院医师。然而,以往研究发现,在住院医师规范化培训学习中过分强调西医知识的学习,医生思维模式“西化”倾向明显,中医经典理论知识薄弱,诊疗过程无中医临证思维模式。此外,由于名医名师影响力较大,门诊量众多,学员很难从跟师过程中体悟到名师的临床思维和诊疗思路。通过实施小型私人在线课程(Small Private Online Course,SPOC)教学,实现线上学习平台教学与线下传统面授教学相融合,形成中医临床思维混合教学模式。其以多种方式加强住院医师规范化培训学员中医临床思维能力培养,可以提高学习效率。可培养学生的自主学习和探索式学习能力,促进教学质量提高,为培养高质量的中医临床人才提供可能,从而达到中医住院医师规范化培训的培养目标。The TCM standardized training of residents is a key link in the reform of TCM talent training,it is an effective way to strengthen the construction of TCM clinical talents.The classics of TCM is the source of the TCM theory and the best teaching material to cultivate the thinking and innovative ability of TCM.In the TCM standardized training of residents,it is necessary to continuously improve the self-confidence of doctors,pay attention to the cultivation of basic theoretical knowledge,clinical syndromeT differentiation thinking and diagnosis and treatment ideas,so as to cultivate qualified TCM residents.However,previous studies have found that there is too much emphasis on the learning of western medical knowledge in the TCM standardized training of residents,the doctor’s thinking mode tends to be westernized obviously,the knowledge of TCM classical theory is weak,and there is no TCM clinical thinking mode in the process of diagnosis and treatment.In addition,in the process of following teachers,due to the heavy workload of teachers,it is difficult for students to understand the clinical thinking and diagnosis and treatment ideas of teachers.Through the implementation of SPOC teaching mode,we can realize the integration of online teaching and face to face teaching,and form a mixed teaching mode of clinical thinking of TCM.It can strengthen the cultivation of TCM clinical thinking ability of doctors in a variety of ways and improve learning efficiency.It can cultivate students’independent learning and exploratory learning ability,promote the improvement of teaching quality,and provide the possibility for cultivating high-quality TCM clinical talents,so as to achieve the training goal of standardized training of TCM residents.
