摘 要:目的 探讨团体箱庭疗法对学龄期注意缺陷多动障碍(ADHD)共患焦虑情绪儿童的临床效果。方法 将2018年6月—2020年6月福建省妇幼保健院收治的60例学龄期ADHD共患焦虑情绪儿童作为研究对象,随机分成两组,对照组30例儿童采取药物治疗方案,观察组30例儿童在药物治疗基础上,接受团体箱庭疗法。治疗前后采用Conners父母症状问卷(PSQ)、注意缺陷多动障碍评定量表父母版(SNAP-Ⅳ)、睡眠习惯问卷(CSHQ)、焦虑自评量表(SAS)及儿童焦虑性情绪障碍筛查量表(SCARED)评价临床效果。结果 干预前,两组儿童PSQ问卷品行问题、学习问题、心身障碍、多动/冲动、焦虑及多动指数因子得分,SNAP-Ⅳ量表注意缺陷、多动冲动因子得分,CSHQ、SAS及SCARED量表总分比较,差异均无统计学意义(均P>0.05)。干预后,观察组儿童PSQ问卷学习问题、心身障碍、多动/冲动、焦虑及多动指数因子得分分别为1.0(1.0,1.3)分、0.3(0.2,0.3)分、1.0(0.9,1.3)分、0.3(0.3,0.5)分、1.1(0.8,1.2)分,SNAP-IV量表注意缺陷、多动冲动因子得分分别为1.0(0.8,1.2)分、1.1(1.0,1.2)分,CSHQ、SAS及SCARED量表总分分别为(35.3±3.5)分、37.5(36.3,45.0)分、15.0(13.0,18.0)分,均低于对照组[1.5(1.0,1.5)分、0.4(0.3,0.6)分、1.3(1.3,1.3)分、0.8(0.5,0.9)分、1.2(1.1,1.2)分、1.2(1.0,1.4)分、1.3(1.2,1.6)分、(40.4±3.2)分、45.0(45.0,45.0)分、20.0(18.0,23.0)分],差异均有统计学意义(均P<0.05)。结论 团体箱庭疗法联合药物治疗可以有效改善ADHD共患焦虑情绪儿童的核心症状、睡眠问题及焦虑情绪,可作为早期干预手段。Objective To explore the clinical effect of group sandplay therapy in treatment of school-age children with attention-deficit hyperactivity disorder(ADHD) complicated with anxiety.Methods Sixty school-age children with ADHD and anxiety treated in Fujian Provincial Maternity and Children's Hospital from June 2018 to June 2020 were selected and randomly divided into two groups.Thirty children in control group were treated with drug therapy,and thirty children in observation group were treated with group sandplay therapy on the basis of drug treatment.Conners Parent Symptom Questionnaire(PSQ),Swanson Nolan and Pelham,Version Ⅳ Scale-parent form(SNAP-Ⅳ),Children's Sleep Habits Questionnaire(CSHQ),Self-Rating Anxiety Scale(SAS),and Screen for Child Anxiety Related Emotional Disorders(SCARED) were used to evaluate the clinical effect.Results Before intervention,there was no statistically significant difference in the scores of behavioral problems,learning problems,psychosomatic disorders,hyperactivity/impulsivity,anxiety,and hyperactivity index of PSQ questionnaire,the scores of attention and hyperactivity impulsivity of SNAP-Ⅳ questionnaire,the total scores of CSHQ questionnaire,SAS questionnaire,and SCARED questionnaire between the two groups(P>0.05).After intervention,the scores of learning problems,psychosomatic disorders,hyperactivity/impulsivity,anxiety,and hyperactivity index of PSQ questionnaire in observation group were 1.0(1.0,1.3),0.3(0.2,0.3),1.0(0.9,1.3),0.3(0.3,0.5),1.1(0.8,1.2),the scores of attention and hyperactivity impulsivity of SNAP-Ⅳ questionnaire were 1.0(0.8,1.2),1.1(1.0,1.2),the total scores of CSHQ questionnaire,SAS questionnaire,and SCARED questionnaire were(35.3±3.5),37.5(36.3,45.0),15.0(13.0,18.0),which were lower than those in control group [1.5(1.0,1.5),0.4(0.3,0.6),1.3(1.3,1.3),0.8(0.5,0.9),1.2(1.1,1.2),1.2(1.0,1.4),1.3(1.2,1.6),(40.4±3.2),45.0(45.0,45.0),20.0(18.0,23.0)分],there were statistically significant differences(P<0.05).Conclusion Group sandplay therapy combined with
