全程导师制在神经外科住院医师规范化培训中的价值

摘　　要：目的探讨全程导师制在神经外科住院医师规范化培训(简称住培)中的效果及价值,为提升住培过程管理质量提供参考。方法选取南方医科大学南方医院神经外科专业基地2014年以来的本单位住院医师和面向社会招收住院医师共18名为研究对象。介绍该院全程导师制选聘办法、工作职责、工作形式、管理与考评、激励与惩罚。统计全程导师指导和帮助住院医师的频次、时长和主要内容。收集施行全程导师制度前后本专业基地住院医师的各科出科考核评分、首次执业医师考核通过率、年度业务水平测试成绩、结业考试成绩以及论文发表数量。结果导师指导住院医师的平均次数为16.6次/年,平均时长为1.0小时/次,内容主要包括专业知识和技能、科研和职业规划。全程导师制度组住院医师的出科考核评分、年度业务水平测试成绩、结业考试成绩以及论文发表数量较非全程导师制度组住院医师有较大提升,差异有统计学意义(均P<0.05)。结论全程导师制对神经外科住培有重要价值,符合住院医师的需求,有益于增强住培效果及导师带教积极性。Objective To understand the effect and value of the full-course tutorial system in the standardized training of neurosurgery residents,and to provide reference for improving the management quality of the residency training process.Methods A total of 18 residents in Nanfang Hospital since 2014,including residents from this hospital and recruited from the society,were selected as the research subjects.The qualifications and selection methods of tutors,responsibilities,working modes,management and evaluation methods,and incentive and punishment measures were introduced detailly.The mean frequency,duration and the main contents of communication between tutors and residents were queried.The training scores of every specialty,the first-time pass rate of the qualification exam,the grade of the annual test,the results of the final exam,and the number of published articles were collected.Results The mean frequency of communication between tutors and residents was 16.6 times/year,and the average duration was 1.0 hours/time.The main guidance content included professional knowledge and skills,scientific research,and career planning.Compared with the residents in the non-full-course tutorial system group,the resident physicians in the full-course tutorial system group had a statistically greater improvement in the training scores of every specialty,the grade of the annual test,the results of the final exam,and the number of published articles(All P<0.05).Conclusion The full-course tutorial system is of great value to the standardized training of neurosurgery residents because it meets the needs of residents and contributes to the improvement of training effect and the enthusiasm of tutors.Therefore,it is worthy of further research,improvement and promotion.
