医学生气道建立的学习在模拟人上的应用效果

摘  要:目的评估不同教学方法对医学生在气道建立学习中的培训效果。方法通过自愿申请方式,选择2019年9~11月兰州大学第二医院的临床医学专业实习学生200名进行院前急救培训,采用随机数字表法将医学生分为传统教学组(n=100)和模拟教学组(n=100),年龄20~28岁,性别不限。传统教学组采用传统教学模式结合模拟人进行培训,模拟教学组采用情景模拟教学结合模拟人进行培训,比较两组医学生进行气道管理操作的成功率以及所需时间的差异。结果传统教学组气管插管、喉罩和食管-气管联合导管置入时间分别为18.66±4.76 min、7.08±1.13 min、6.47±1.65 min,明显高于模拟教学组所需操作时间(16.71±3.30 min、6.11±1.34 min、5.78±1.53 min),模拟教学组在气管插管和喉罩置入操作上具有更高的成功率和更短的操作时间(P<0.01)。两组在置入食管-气管联合导管的成功率均为100%,但模拟教学组的操作时间更短(P<0.05)。结论模拟教学的应用,能够提升医学生院前急救培训中气道建立的能力,培训效果明显提高。Objective To evaluate the training effect of different teaching methods on training of airway establishment for medical students.Methods Through voluntary application,200 clinical medical interns in Lanzhou University Second Hospital,from September to November 2019,were selected for prehospital emergency care.These interns of either gender,aged 20-28 years were divided into the traditional teaching group(n=100)and the simulation teaching group(n=100),according to random number table method.The traditional teaching group received training through a conventional teaching model combined with advanced simulated human,while the simulation teaching group used simulation-based teaching combined with simulated human.The success rates and the time required for airway management were compared between the two groups.Results The time required for endotracheal intubation,laryngeal mask insertion,and esophageal-tracheal tube placement in the traditional teaching group were 18.66±4.76 min,7.08±1.13 min,6.47±1.65 min,respectively,which was significantly longer than those in the simulaion group(16.71±3.30 min,6.11±1.34 min,5.78±1.53 min).The simulation group demonstrated higher success rates and shorter time for endotracheal intubation and laryngeamask insertion(P<0.01).Both groups achieved a 100%success rate for esophageal-tracheal tube placement,but the simulation group had a significantly shorter procedure time(P<0.05).Conclusion Simulation-based teaching significanty enhances medical students'proficiency in airway management during pre-hospital emergency training.
