摘  要:目的探索在异常分娩教学查房中应用高仿真模型解决产程的动态变化。方法选取2021年9月至2023年6月期间在重庆大学附属三峡医院妇产科专业基地的二年级和三年级住院医师作为研究对象,随机分为模拟教学组(观察组)和传统教学组(对照组)。观察组异常分娩教学查房利用高仿真模型,直观胎儿在产力下为适应产道发生的动态变化,复盘产力、产道、胎儿相互影响及产程经过,并结合产程图分析讨论;对照组利用幻灯片和挂图结合产程图对病例分析讨论。教学查房结束后,利用李克特量表评价住院医师对两种教学查房模式的主观体验,开展理论考试和技能考核,比较两组成绩差异。结果观察组体验更逼真、自然,认为有利于建立住院医师的临床思维,将理论与临床实际相结合,能提高住院医师的医患沟通和团队合作等岗位能力。结论异常分娩教学查房中应用高仿真模型能帮助住院医师更好理解和分析产程中产力、产道、胎儿相互影响及产程经过,寻找异常分娩的病因。Objective To explore the application of high simulation models in abnormal delivery teaching rounds to address the dynamic changes in the labor process.Methods The second year and third year resident physicians at the Obstetrics and Gynecology Professional Base of Chongqing University Affiliated Three Gorges Hospital from September 2021 to June 2023 were selected as the research subjects.They were randomly divided into a simulated teaching group(observation group)and a traditional teaching group(control group).The observation group used a high simulation model for abnormal delivery teaching rounds to visualize the dynamic changes of the fetus under labor force to adapt to the changes in the birth canal.The study reviewed the labor force,birth canal,fetal interaction,and labor process,and analyzed and discussed them in combination with the labor process diagram;The control group used PPT and wall charts combined with labor process diagrams to analyze and discuss the cases.After the teaching rounds were completed,the Likert scale was used to evaluate the subjective experience of resident physicians towards the two teaching rounds,and theoretical and skill assessments are conducted to compare the differences in scores between the two groups.ResultsTThe Likert scale and assessment results showed that the observation group had a more realistic and natural experience,which was believed to be beneficial for establishing the clinical thinking of resident physicians,combining theory with clinical practice,and improving their abilities in doctor-patient communication and teamwork.Conclusion The application of high simulation models in teaching ward rounds for abnormal childbirth can help resident physicians better understand and analyze the interaction between labor,birth canal,fetus,and labor process,and identify the causes of abnormal childbirth.
