摘 要:目的探讨置信职业行为(entrustable professional activities,EPAs)的教学评价体系在甲状腺外科住院医师规范化培训(简称住培)中的可行性及效果。方法选取2022年9月至2023年12月于浙江大学医学院附属第一医院甲状腺外科参加住培的30名外科住院医师开展EPAs教学及置信等级评估。比较住院医师每项EPA初末两次预期置信等级评估达标率,以及不同年级整体EPA项目达标率,分析EPAs调查问卷表。结果住院医师总体末次EPAs评估达标率高于初次EPAs评估达标率(P<0.05)。住院医师分层置信评估表明,不同年级住院医师初末两次评估结果比较差异有统计学意义(P<0.05),且住院医师的年级越高,EPAs预期置信等级的整体达标率越高。调查问卷结果表明,绝大部分住院医师对EPAs教学效果表示肯定。结论EPAs的教学评估模式能综合提高甲状腺外科住院医师的预期置信等级以及核心胜任力水平,可为基于胜任力导向的住培与评价提供一定的理论依据。Objective To explore the feasibility and effective evaluation of the teaching evaluation system based on entrustable professional activities(EPAs)in the standardized training of thyroid surgical residents.Methods From September 2022 to December 2023,30 thyroid surgical residents from the First Hospital of Zhejiang University School of Medicine were selected to implement the teaching and entrustable level assessment of EPAs.We statistically analyzed the pass rate of the initial and final entrustable level assessment and the overall pass rate of the EPAs program in different grades.EPAs questionnaire form was analyzed.Results The pass rate of the final EPAs assessment was higher than the initial EPAs assessment(P<0.05).The results of the resident's entrustable assessment indicated the overall pass rate of entrustable assessment for EPAs was positively correlated with the resident's grade(P<0.05).And the higher the grade of the residents,the higher the overall compliance rate of the expected confidence level of EPAs.The survey results indicate that the vast majority of resident physicians affirm the effectiveness of EPA teaching.Conclusion The teaching evaluation model of EPAs can comprehensively improve the anticipated entrustable level and the core competency level of thyroid surgical residents,which provides a certain theoretical basis for competency-oriented residents training and evaluation.
