摘 要:目的:现代急诊医学的发展和人口老龄化的加剧对急诊护士的能力提出了更高的要求,急诊护士的核心能力是保障患者安全和提高护理质量的关键。本研究旨在对急诊护士核心能力要素的质性研究结果进行Meta整合。方法:计算机检索WebofScience、 PubMed、 CINAHL、 Embase、 TheCochraneLibrary、 Ovid、 Medline、PsycINFO、中国知网、万方数据库、维普数据库和中国生物医学文献数据库中关于急诊护士核心能力要素的质性研究,检索时间范围为建库至2024年9月。进行文献质量评价后,采用澳大利亚乔安娜布里格斯研究所(Joanna Briggs Institute,JBI)循循证卫生保健中心Meta整合中的汇集性整合法对结果进行整合。结果:共获得1 383篇相关文献,经严格筛选后最终纳入11项研究,包含1篇中文文献和10篇英文文献。通过对纳入文献的数据提取,共归纳出46个主要研究结果。文献质量评价显示:1篇研究为A级证据,10篇为B级证据。基于ConQual评级系统,本研究的整体证据可信度处于中等水平。通过Meta整合分析,将急诊护士核心能力归纳为8个新类别,并进一步凝练成临床实践、心理复原和角色发展3个方面的能力。结论:急诊护士需要具备临床实践、心理复原和角色发展多维度的核心能力,管理者应重视并加强对这些核心能力的培养,以期提高急诊护士的综合素质。Objective:The advancement of modern emergency medicine and the intensifying aging population have raised the bar for the capabilities of emergency nurses.Core competencies are essential for ensuring patient safety and improving the quality of care.This study aims to conduct a Meta-integration of qualitative research findings regarding the core competency elements of emergency nurses.Methods:A comprehensive literature search was conducted in Web of Science,PubMed,CINAHL,Embase,The Cochrane Library,Ovid,Medline,PsycINFO,CNKI,Wanfang Data,VIP Database,and CBM from inception to September 2024.Following a quality appraisal of the literature,the Joanna Briggs Institute(JBI)Meta-integration method was applied to integrate the findings.Results:A total of 1383 relevant articles were retrieved,of which 11 studies(1 in Chinese and 10 in English)met the inclusion criteria.From these studies,46 main findings were extracted.Quality assessment rated 1 study as Level A evidence and 10 as Level B.According to the ConQual system,the overall confidence level of the synthesized evidence was moderate.The Meta-integration identified 8 new categories,which were further condensed into 3 integrated domains of core competencies:Clinical practice,psychological resilience,and role development.Conclusion:Emergency nurses require multidimensional core competencies encompassing clinical practice,psychological resilience,and role development.Healthcare administrators should prioritize and enhance the cultivation of these core capabilities to improve the comprehensive quality and performance of emergency nursing staff.
