摘  要:目的探讨问题教学法(problem-based learning,PBL)联合循证医学模式(evidence-based medicine,EBM)在胆囊结石微创手术治疗临床带教中的效果。方法选取2021年1—12月于九江市第一人民医院肝胆外科行住院医师规范化培训(住培)的50名住院医师作为研究对象,按照随机数字表法分为对照组与观察组,每组25名。对照组采用传统教学法,观察组给予PBL联合EBM教学。比较两组学习成绩、教学质量、临床实践技能、核心能力、批判性思维、教学满意度。结果观察组理论知识、操作技能得分均高于对照组,差异有统计学意义(P<0.05)。观察组教学方法、硬件水平、教学态度、学习能力、医德树立评分均高于对照组,差异有统计学意义(P<0.05)。观察组主观评价评分、肝脏脉管解剖理解评分、肝段解剖辨识评分、病灶解剖位置分析评分、模拟手术规划评分及总分均高于对照组,差异有统计学意义(P<0.05)。教学结束后,观察组批判性思维及科研、人际关系、专业发展、伦理与法律实践、教育咨询评分均高于对照组,差异有统计学意义(P<0.05)。教学结束后,观察组寻找真相、分析能力、开放思想、求知欲、系统能力、批判性信心、认知成熟性评分均高于对照组,差异有统计学意义(P<0.05)。观察组教学满意度高于对照组,差异有统计学意义(P<0.05)。结论PBL联合EBM教学法可提高胆囊结石微创手术治疗的临床带教教学质量,培养住院医师的批判性思维能力,增强住院医师的接受程度。Objective To explore the effect of problem-based learning(PBL)combined with evidence based medicine(EBM)in clinical teaching of minimally invasive surgery for gallstones.Methods 50 residents who received standardized resident training(resident training)from the Hepatobiliary Surgery department of the NO.1 People's Hospital from January to December 2021 were selected as the research subjects,they were divided into the control group and the observation group according random number table method,with 25 trainees in each group.The control group received traditional teaching methods,while the observation group received PBL combined with EBM teaching.The academic performance,teaching quality,clinical practice skills,core competencies,critical thinking,and teaching satisfaction were compared between the two groups.Results The theoretical knowledge and operational skills scores in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The teaching methods,hardware level,teaching attitude,learning ability,and medical ethics establishment scores in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The subjective evaluation score,liver vascular anatomy understanding score,liver segment anatomy identification score,lesion anatomical location analysis score,simulated surgical planning score and total score in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After the teaching ended,the scores of critical thinking,scientific research,interpersonal relationships,professional development,ethical and legal practice and educational counseling in the observation group were higher than the control group,and the differences were statistically significant(P<0.05).After the teaching ended,the scores of seeking truth,analytical ability,open mindedness,thirst for knowledge,systematic ability,critical confidence,and cogni
