摘 要:目的 观察标准化病人(SP)联合迷你临床演练评估(Mini-CEX)教学模式在全科住院医师规范化培训(简称住培)中的应用。方法 选取全科住院医师100人,随机分成2组,观察组50人,采取SP联合Mini-CEX;对照组50人,采取传统教学。观察两组出科时Mini-CEX量表评分、理论知识成绩、临床技能成绩以及SOAP病例书写成绩,同时调查两组住院医师对教学模式的认可程度。结果 观察组Mini-CEX量表评分高于对照组,特别是人文关怀和沟通技巧(P<0.05);观察组出科时理论知识成绩高于对照组,但差异无统计学意义(P> 0.05),临床技能成绩和SOAP病例书写成绩均高于对照组(均P<0.05);观察组对教学模式认可程度高于对照组(均P<0.05)。结论 标准化病人联合Mini-CEX教学模式有效提高了住院医师临床能力、医学人文和医患沟通,深受住院医师喜欢,在全科住培中的有一定的应用价值,值得进一步推广。Objective To observe the application of standardized patients(SP)combined with mini clinical exercise evaluation(Mini-CEX)teaching mode in standardized residency training of general practice.Methods A total of 100 general practitioners were randomly divided into two groups,with 50 in the observation group adopting the teaching mode of SP combined with Mini-CEX and 50 in the control group(adopting the traditional teaching mode).Scores of the Mini-CEX scale,the theoretical knowledge,the clinical skills and SOAP of the two groups were compared.Moreover,the degree of recognition of the teaching mode by the residents of the two groups was investigated.Results The scores of the Mini-CEX scale in the observation group were higher than those in the control group(P<0.001),especially in terms of humanistic care and communication skills(P<0.001).The scores of theoretical knowledge in the observation group were slightly higher than those in the control group(P>0.05).The scores of clinical skills and SOAP in the observation group were higher than those in the control group(P<0.05).The scores of degree of recognition of the teaching mode by the residents in the observation group were higher than those in the control group(all P<0.001),with statistically significant differences.Conclusion The teaching mode of standard patients combined with Mini-CEX can effectively improve the clinical abilities,medical humanities and doctor-patient communication of resident physicians,and is preferred by residents.Thus it has certain practical value in standardized residency training of general practice and is deeply worth further promotion.
