临床实境考核在眼科住院医师实践技能结业考核中的应用

摘　　要：目的评价临床实境考核在眼科住院医师规范化培训实践技能结业考核中的应用效果。方法将湖北省2016年和2017年参加眼科专业住院医师规范化培训实践技能结业考核的住院医师63名作为研究对象,其中将2016年参与的28名住院医师设为对照组,将2017年参与的35名住院医师设为研究组。比较临床实境考核(研究组)和传统模拟考核(对照组)的平均成绩和考生反馈的差异。结果在考核成绩方面,研究组住院医师的手术操作平均成绩为(84.93±1.53)分,显著低于对照组的(96.82±0.64)分(P<0.001);在辅助检查、问诊、体格检查、病历书写平均成绩方面,两组住院医师比较差异均无统计学意义(P>0.05);在考核内容难易度、站点时间充足、考核适应性的满意度方面,研究组住院医师的评分均低于对照组(P<0.05);在反映真实能力水平的满意度方面,研究组住院医师的数据为85.71%,显著高于对照组的64.29%(P<0.05)。结论临床实境考核比传统的模拟考核更能有效评估眼科住院医师的手术操作能力。ObjectiveTo assess the applicability of real-case-based tests in the final examination of ophthalmology residents.Methods The sixty-three subjects were selected from ophthalmology residents from Hubei Province who took the residency final examination in2016 and 2017 respectively. The twenty-eight subjects who took the traditional simulated test in 2016 were assigned to the control group while the rest of the subjects who took the real-case-based test in 2017 were assigned to the research group. The scores and feedback were compared between the two groups.ResultsThe scores of operation skills in the research group(84. 93±1. 53) were significantly lower than those of the control group(96. 82±0. 64)(P<0. 001), but there was no significant difference in other items. The scores of satisfaction with the levels of difficulty of the test, time allocation and adaptability to the test were lower in the research group than in the control group(P<0. 05), but the levels of satisfaction with the effectiveness of the test were higher in the research group 85. 71%(30/35)than in the control group 64. 29%(18/28).ConclusionThe real-case-based test could more effectively assess the operation skills of ophthalmology residents than the traditional simulated practice.
