新形势下住院医师临床实践能力的全过程管理

摘　　要：临床实践能力是住院医师培训和过程考核的主要内容,但住院医师基本技能操作水平及临床思维参差不齐,带教教师往往重视理论知识传授,而忽视了规范的基本技能操作、医患沟通、医德医风、临床思维等临床实践能力的传授,导致岗位胜任力提升较慢,提高在培住院医师临床实践能力十分重要。针对临床基本功和临床实践能力的不足的突出问题,围绕如何提高在培住院医师临床实践能力,建立培训前期-培训中期-培训后期全过程管理模式,培训前期加强岗前培训,培训中期加强能力培养,包括重视日常轮转安排及出科考核、重视执业医师考核和临床技能竞赛、重视院级督导保障教学质量、重视“住培小教师”传帮带,培训后期重视结业考核,重视阶段考核及技能比赛,完善“评价-反馈-奖惩”督导体系,教学相长,在提高住院医师岗位胜任力取得了明显的成效。Clinical practice ability is the main content of resident training and process assessment,but the basic skill operation level and clinical thinking of resident are uneven.Teachers often attach importance to the teaching of theoretical knowledge,often ignore the teaching of standardized basic skill operation,doctor-patient communication,medical ethics,clinical thinking and other clinical practice ability,resulting in the promotion of post competency slowly and it is very important to train resident's clinical practice ability.In view of the outstanding problems of the deficiency of clinical basic skills and clinical practice ability,we focus on how to improve the clinical practice ability of resident doctors in training,establish the whole process management mode from the early stage of training to the middle stage of training to the later stage of training,strengthen the pre post training in the early stage of training,and strengthen the ability training in the middle stage of training,including paying attention to the daily rotation arrangement and graduation examination,paying attention to the examination of licensed doctors and clinical practice In the later stage of training,we should pay attention to the completion assessment,stage assessment and skill competition,improve the evaluation feedback reward and punishment supervision system,and improve the ability of teaching and learning.
