摘  要:目的将案例教学法结合翻转课堂教学模式应用在神经感染临床带教中,探讨其教学效果。方法选取2019年3月1日-2020年9月30日在首都医科大学某医院神经内科感染组学习的进修医师作为研究对象;其中2019级24人为对照组,采取传统带教模式;2020级20人为观察组,采用案例教学法结合翻转课堂教学模式。采用简易临床评估量表(Mini-CEX量表)及自主设计调查问卷对两组教学效果进行对比。结果观察组出科时Mini-CEX量表问诊技能、体检技能、人文关怀、临床判断、沟通技能、组织效能、整体临床胜任能力为(8.25±0.72)分、(8.00±0.73)分、(7.29±0.90)分、(7.71±0.72)分、(7.29±1.06)分、(8.19±0.98)分和(8.25±0.64)分,较对照组均提高,差异均具有统计学意义(P<0.05);对自主设计的调查问卷进行匿名打分,观察组在提高学习兴趣、提高自学能力、培养临床思维、提高分析/解决问题的能力、提高医患沟通能力、增加团队协作方面的得分均高于对照组,差异具有统计学意义(P<0.05)。结论案例教学法结合翻转课堂教学模式可显著提高进修医师对神经感染的学习兴趣和自学能力,有助于培养临床思维、提高分析解决问题的能力,增加团队协作精神,提升学员满意度及学习体会,具有推广价值。Objectives This study aims to explore the effect of case teaching method combined with flipped classroom teaching mode in clinical teaching of nerve infection.Methods Training physicians who were studying in the Infection Group of Department of Neurology,Xuanwu Hospital,Capital Medical University from March 1,2019to September 30,2020 were selected as the research objects.Among them,24 students in the class of 2019 in the control group adopted the traditional teaching mode and 20 students in the class of 2020 in the observation group adopted the case teaching method combined with flipped classroom teaching mode.A simple clinical assessment scale(Mini-CEX scale)and a self-designed questionnaire were used to compare the teaching effects of the two groups.Results After training,the scores of the observation group in the Mini-CEX scale assessment form from the aspects of consultation skills,physical examination skills,humanistic care,clinical judgment,communication skills,organizational effectiveness and overall clinical competence were 8.25±0.72,8.00±0.73,7.29±0.90,7.71±0.72,7.29±1.06,8.19±0.98and 8.25±0.64respectively,which were all significantly higher than those in the control group(P<0.05).The self-designed questionnaire was scored anonymously.The result showed that the observation group scored significantly higher than the control group from the perspectives of improving learning interest,improving self-learning ability,cultivating clinical thinking,improving analysis and problem-solving ability,improving doctor-patient communication ability,and increasing teamwork(P<0.05).Conclusions The combination of case teaching method and flipped classroom teaching mode can significantly improve the learning interest of the training physicians on nerve infection,improve their self-learning ability,helps develop clinical thinking and improve analytical problem-solving skills,increase teamwork spirit,and improve student satisfaction and learning experience.This teaching mode is worth being promoted.
