摘  要:临床思维培养是基于胜任力导向住院医师规范化培训(简称住培)的重要内容。本研究应用ADDIE模型建立并探索完善儿科住院医师临床思维课程。首先针对现有住院医师课程存在的问题以及临床思维能力培养的迫切需要进行了需求分析和可行性评估,然后进一步设计、开发并实施了小规模限制性在线课程与案例为基础的教学相结合的儿科住院医师临床思维课程体系,涵盖了儿科住培中要求掌握的各系统主要内容。课程结束后的调查问卷显示所有参与学习的住院医师都认可本门课程,认为对于临床思维能力培养非常有益。根据住院医师的建议以及教学需求,第二年对课程进行了再次开发与改进,将以团队为基础的学习方法融入课程体系中。总之,应用ADDIE模型有机整合多种教学模式,对于改进儿科住院医师临床思维课程具有比较高效的推动作用,在今后的教学体系改革中值得借鉴。Clinical thinking training is an important part of competency-based standardized training for residents.The purpose of this study was to construct and improve clinical thinking curriculum for pediatric residents by applying ADDIE model(Analysis,Design,Development,Implementation,and Evaluation).Firstly,through interviews with residents and teachers,combined with the current situation of the professional training base,needs analysis and feasibility evaluation was conducted.Next,we designed,developed and implemented a pediatric resident clinical thinking curriculum system combining Small Private Online Courses(SPOC)and Case-Based Learning(CBL)teaching.The curriculum covered the main contents of each system required to be mastered in the standardized training of pediatric residents.Each lesson included pre-class preparation,CBL in class,homework and feedback after class.The questionnaires after the curriculum showed that all residents participating in the study approved the course and believed that the course was beneficial to the cultivation of clinical thinking ability.According to the teaching needs and the suggestions of the residents,the course was re-developed and improved in the follow-up,and Team-Based Learning(TBL)teaching was integrated into the curriculum system.In conclusion,the application of the ADDIE model,the organic integration of multiple teaching modes,and the construction and improvement of clinical thinking courses for pediatric residents are in line with the current resident training needs and are worthy of further promotion.
